RESIDENT HAIRCUT SERVICES FY 2020 SCOPE: Contractor shall provide haircuts for all residents of the Community Living Center (CLC) Units Alpha and Beta and the Psychosocial Residential Rehabilitation Treatment Units at the Erie VA Medical Center, 135 East 38th Street, Erie, PA 16504. Contractor plans, organizes, and performs haircuts on a monthly basis to the residents of the Community Living Center, Alpha and Beta, and the Psychosocial Residential Rehabilitation Treatment Program (PRRTP). Contractor will provide services three times a month, once for haircuts on CLC Units Alpha, once for haircuts on CLC Unit Beta, and once for haircuts on the PRRTP Unit. Haircuts are provided for long term care, hospice, and PRRTP residents only. Prior to visits, the Recreation Therapist for each unit (Units Alpha and Beta) and a staff member from PRRTP will compile of list of residents who will receive a haircut and provide this to the contractor. The Shower/Whirlpool Rooms on Units Alpha and Beta and the PRRTP Unit will be the designated haircut room. Unit Alpha, Unit Beta, and PRRTP staff are responsible for transporting residents to and from the designated room on each unit for haircuts. Contractor will bring all needed equipment and disinfecting agents and follow infection control guidelines while performing haircuts. Contractor will keep a record of residents who have received haircuts at each visit. This record will be submitted to the Recreation Therapist of each CLC unit and a PRRTP staff member on the PRRTP unit at the end of each visit. QUALIFICATIONS: Contractor must possess a current Professional Cosmetology or Barber License for the state of Pennsylvania. Documentation of license must be provided to Hospice/Palliative Care Coordinator. LOCATION OF DUTIES: Whirlpool/Shower Room on Units Alpha and Beta and PRRTP Unit Erie VA Medical Center, 135 East 38th Street, Erie, PA 16504 GOVERNMENT FURNISHED PROPERTY AND SERVICES: Erie VAMC will provide an area to perform resident haircuts. This location is the Whirlpool/Shower Room on CLC Units Alpha and Beta and PRRTP Unit. Contractor will bring all needed equipment and necessary disinfecting supplies. Upon completion of haircuts, Contractor will clean up and arrange the area to its original set up following haircut completions. TIME AND FREQUENCY OF DUTIES: Monthly: Approximately 8-15 haircuts per visit to CLC Unit Alpha and Unit Beta will be completed. A maximum of 8 haircuts per visit to PRRTP Unit will be completed. Visit time will be approximately 2.5-3.5 hours per visit on each CLC Unit and PRRTP Unit. The Contractor will establish a schedule for haircut visits with the Recreation Therapist on CLC Unit Alpha and CLC Unit Beta and a PRRTP staff member on the PRRTP unit at the Erie VAMC. Once the schedule is established, all haircuts will occur on the agreed upon schedule. If the Contractor is unable to perform the above duties due to illness or emergency, Contractor must notify the respective CLC Unit Recreation Therapist (or designee) and PRRTP Unit staff member at the Erie VAMC at their earliest possible convenience. SECURITY POLICE CHECK-IN: All contractor personnel working onsite at the VA facility shall sign in and out with VA Police dispatch located in the front entrance of the main hospital building. While on VA Medical Center premises, Contractor employees will wear a visible identification badge which contains a photograph of the employee. HEALTH REQUIREMENTS: Contractor must meet the Erie VAMC Health Requirements as approved by the Erie Employee Occupational Health Nurse.
