MUSIC THERAPY PROGRAM COMMUNITY LIVING CENTER FY 2020 VA MEDICAL CENTER-ERIE, PA Contractor shall provide a Music Therapy Program for appropriate residents of the Community Living Center (CLC) Units Alpha and Beta at the Erie VA Medical Center, 135 East 38th Street, Erie, PA 16504. Contractor will bring all needed equipment and disinfecting agents and follow infection control guidelines while performing Music Therapy. QUALIFICATIONS: Contractor must possess a Master of Art in Music Therapy. Documentation of degree must be provided to the Contracting Officer s Representative (COR). LOCATION OF DUTIES: Group Music Therapy programs in the resident dining room or individual resident rooms on the CLC Units Alpha and Beta, Erie VA Medical Center, 135 East 38th Street, Erie, PA 16504 GOVERNMENT FURNISHED PROPERTY AND SERVICES: Erie VAMC will provide an area to perform music Therapy and provide musical instruments such as a piano, organ, keyboard, guitar, and rhythmic instruments to be used during musical therapy sessions. Music Therapist may bring their own guitar. The Music Therapist is not permitted to bring other instruments in to the Erie VAMC for use by residents. The Erie VAMC CLC will provide the necessary equipment and supplies in accordance with infection control needs as identified. TIME AND FREQUENCY OF DUTIES: Weekly: Contractor will provide Music Therapy Services-one designated time for each CLC Unit Alpha and Beta. Visit time will be approximately 2.0 hours per visit on each individual CLC Unit. The Music Therapist will establish a schedule for Music Therapy visits with the Recreation Therapist on CLC Unit Alpha and CLC Unit Beta. Once the schedule is established, all Music Therapy visits will occur on the agreed upon schedule. PRICE COST: MUSIC THERAPY for CLC Units Alpha and Beta: hr. X 4 hours X 52 weeks = per contract year ($ per CLC Unit)
