Pittsburgh VAMC, Health Care, is seeking SDVOSB/VOSB and any SBC regardless of Socio-economic category that can provide the Simbionix LapMENTOR virtual reality trainer. This requirement will be brand name only, the manufacturer is 3D Systems, part number LP10100. The sources sought is seeking dealers or distributors who can provide the requirement above. The vendors responding must be certified by 3D Systems to deliver, uninstall present equipment, and install the new trainers. (Trade In discount may be required). The trainers are to be purchased for the VA Pittsburgh Healthcare where the system will be installed in BLDG 29, 2nd floor, room 2M312. Refer to the trainer characteristics below. The veteran business must be registered in SAM and certified in VETBIZ (VIP). The small businesses must be registered in SAM. All Vendors interested in, and with the capability and capacity to fulfill this requirement, are required to furnish the following minimum information in reply to this sources sought: Company Name, address, Government Business Point of Contact, telephone number and email address. Company DUNS number, CAGE Code, and Tax ID numbers. Identify any current Federal Supply Schedule, GSA Contracts held, if none then state that no FSS/GSA schedules are held.
