Page 1 of Page 3 of 3 Page 1 of Purpose and Objectives: The intent of this Sources Sought Notice is to identify potential Small Businesses especially any SDVOSB offerors capable of providing the items listed below: These are for the Lebanon VA Medical Center in Lebanon, PA 17042. Responses to this Sources Sought Notice should demonstrate the firm's ability, capability, and responsibility to provide the principal components of supplies listed in the below Statement of Need. Responses should include the following information: Business name, address, Business point of Contact. All information is to be submitted via e-mail at Ronald.Kline@va.gov. Information provided will not be returned. All responses shall be in the English Language. Responses are due by 12:00 pm (EST) on Thursday, January 31, 2019. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. This notice does not constitute a formal Request for Quote (RFQ), nor is the government obligated to issue an RFQ. STATEMENT OF NEED TOTAL JOINT SURGICAL INSTURMENT KIT General Equipment Name: 65 Surgical Instruments Description and Salient Characteristics: The Lebanon VA Medical Center (VAMC), has a requirement for ONE (1) Zimmer brand name or equal TM TIBIA-PTT441 surgical instrument sets left/right. These instrument sets are used for total joint replacement surgeries performed at the Lebanon VAMC. All instruments and accessories must be compatible with Zimmer brand equipment currently in use. The Lebanon VAMC stocks and uses Zimmer implants, and requires Zimmer brand or equal instruments to assure compatibility with the prosthetic devices that are used. The instrument set shall be brand new and in original packaging. Sterilization and processing instructions are required upon delivery. All products are manufactured by Zimmer, Inc.
