STATEMENT OF WORK (SOW): Work performed under this scope will be for and conducted within the VA Western New York Healthcare System (VAWNYHS) at the Buffalo VA Medical Center (VAMC) located at 3495 Bailey Avenue Buffalo, NY 14215. This Statement of Work is for Parking Lot Sealcoating, Crack Sealing, and Striping at the Buffalo VAMC that shall be awarded under a firm fixed price contract and to be completed NLT October 1, 2025. BACKGROUND: The parking lot at the Buffalo VAMC has existing striping that is heavily faded, which causes confusion for employees, patients, and visitors. The government requires sealcoating, crack sealing, and re-striping before winter 2025 starts. SCOPE: Reference drawing PARKING LOT SEALING and STRIPING Dated: 2025-05-05 for all work done in this SOW. Contractor shall be responsible to complete pavement painting (driveways, parking lot striping, pedestrian crosswalks, stop bars, turn arrows, curbs, and other areas detailed on drawings). The size, shape and configuration of all pavement markings shall match existing, attached drawing, or as approved by the VA. Clean and sealcoat all existing pavement cracks measuring 1/8" or greater in width using a hot applied polymeric joint sealant (wr meadows hi-spec or approved equal). Sealcoat E-Lot in its entirety. Re-stripe the parking lot spaces to match existing layout where indicated, and new parking lot space configurations as shown on drawings. Ensure all areas to receive paint are clean. Existing yellow curbs within the work area shall be re-aligned, power washed, and repainted yellow. Apply uniformly painted and reflected pavement marking of required colors, length and width with true sharp edges and ensure all surfaces are properly cured, dry and prepared prior to application of paint. All markings except for curbs shall have reflective glass beads evenly dispersed them while wet at a rate of 6 pounds per gallon. CONTRACTOR QUALIFICATIONS: The contractor shall have experience re-striping and repairing signs of campuses in similar size and complexity. The contractor shall be capable of performing the striping portion on off hours and the weekend. GUIDING DOCUMENTS: All work performed under this contract shall be done in accordance with the following VHA specifications: 01 00 00 PLSS GENERAL REQUIREMENTS 01 32 16.15 PLSS PROJECT SCHEDULES (SMALL PROJECTS) 01 35 26 PLS SAFETY REQUIREMENTS 32 17 23 PLS PAVEMENT MARKINGS SUBMITTALS: In accordance with the terms of PG-18-1 MASTER CONSTRUCTION SPECIFICATIONS; the contractor shall provide submittals for review prior to starting work. A submittal log has been generated for this effort and represents a list of the minimum required submittals based upon the current construction documents. Although every attempt has been made to ensure that this list is complete, additional submittals not listed below may be required. The Medical Center must maintain its operation 24 hours a day 7 days a week. Therefore, all work needs to be done off hours and on the weekend which shall be scheduled and coordinated with the COR to ensure that no lapses in operation occur. Everything outlined in this SOW shall be completed before October 1, 2025. STATUS UPDATES: The Contractor shall prepare and submit status reports throughout the duration of the contract. The frequency of status reports will be approximately once per month, or at the request of the project COR. The reporting period will begin at Notice to Proceed. The contents of status reports shall include but is not limited to the following information/topics: Summary of Completed Works and Current Status of Project Progress Anticipated (for the next 4 weeks) Updated Schedule Problems Encountered and Anticipated Status reports may be organized in Contractor specified format. Status reports will be supplied as a digital product to the Government in a Microsoft Word compatible format (*.DOC, *.DOCX) or Portable Document Format (*.PDF). Status reports shall be submitted via email to the CO and COR. RISK CONTROL: The Contractor shall take all precautions necessary to protect persons and property from injury or damage during performance of this contract. The Contractor shall be responsible for any injury to any Contractor Representative, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by the contractor s fault or negligence. QUALITY ASSURANCE: The Government may inspect each task as completed. The Contractor shall be notified within 3 duty days of unacceptable service. The Contractor shall re-perform service within 15 duty days of notification of unacceptable service. RECORDS MANAGEMENT: A complete written report in PDF format of the inspections performed shall be provided to the COR within five (5) business days of completion. All reference materials shall be included in the electronic version of the report, either through scanning or other means of electronic text import methods. Information outlined in the report shall include the following: Company s name, address, and telephone number. VA contract number, name of COR, and name of Contracting Officer. Date and time of work. Any photographs taken for documentation purposes. Description of inspection findings. Reference materials used to perform inspections. Recommended corrective actions, if any. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. The Government Agency owns the rights to all data/records produced as part of this contract. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format (paper, electronic, etc.) or mode of transmission (email, fax, etc.) or state of completion (draft, final, etc.). No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. Contractor is required to obtain the Contracting Officer s approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information. IDENTIFICATION, PARKING, SMOKING, AND VAMC REGULATIONS/KEY CONTROL All Contractor employees who have access to VA facilities are subject to the same level of investigation as VA employees. Security and badging process to be determined by COR. Contractor employees shall always wear visible identification while on the premises of the VAMC. It is the responsibility of the vendor to park in the appropriate designated parking areas. Information on parking is available from the VA Police Section. The VAMC will not invalidate or make reimbursement of parking violations of the vendor under any conditions. Smoking is prohibited on VAMC property to include inside privately owned vehicles (POV) parked on VAMC property. Smoking shall only be allowed off VAMC property. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court.
