The following verbiage is hereby added to update PWS section 8.5.1 REFERRAL/ADMISSION per recent VHA policy change dated October, 23, 2023: Contractors must ensure timely, same-day whenever possible, admissions to their program in accordance with VA policy. Program admission is defined as the time from when the referred Veteran is ready to enter transitional housing to the actual placement in an available bed with the goal of same-day admission or no more than 72 hours. If a Veteran is directly admitted to the Contractor s facility prior to engagement by VA homeless program staff, the Homeless Operations Management and Evaluation System (HOMES) assessment Interview must be completed within 14 days of admission, consistent with existing CRS and HOMES policies (HOMES Reporting Policy). VA staff may complete parts of the admission process to expedite admissions. Per VHA Directive 1162.04 CRS, HCHV liaisons must verify Veteran s eligibility for program participation within one business day from admission. In the event, that a Veteran is admitted to an HCHV CRS program but subsequently found to be ineligible, the HCHV CRS liaison must immediately work with the Veteran to secure other placement within available VA or community programs. VA will pay for a maximum of four days from the day of admission to allow the provider and HCHV CRS liaison time to locate and arrange an alternate placement.

The Department of Veterans Affairs, Network Contracting Office (NCO) 2, and the VA Upstate New York Healthcare System VA Medical Centers intends to issue solicitation number 36C24223R0003 for Contract Residential Housing Services for the major sites that include the full catchment areas of the Albany VA Medical Center (VAMC), Buffalo VAMC (including the Batavia VAMC)), Canandaigua VAMC (including the Rochester VA Outpatient Clinic (ROPC)), and the Syracuse VAMC. The scope of this procurement is for Contract Residential Housing services as part of its community based Health Care for Homeless Veterans (HCHV) program. The Department of Veteran Affairs (VA) requires contractor(s) to provide emergency residential housing and case management services, in currently active residential treatment facilities, to homeless Veterans as part of the Community Based Health Care for Homeless Veterans (HCHV) program. The facility needs to be currently operating in order to be initially inspected by the VA to ensure operations comply with Federal law and regulations and HCHV inspection standards in accordance with VHA Directive 1162.04. The goal of the HCHV is to remove homeless Veterans from the street or habitation unfit for Veterans and place them in a community-based residential environment with sufficient supportive services to meet their needs and ultimately facilitate the improvement of their housing situation. The resultant contract will be a multiple award, firm fixed price (FP) IDIQ contract for a base period of 1 year, plus 4 one year option periods. The solicitation will be issued at the Contract Opportunities website (https://www.sam.gov) on/about 7/19/2023. The North American Industry Classification System Code (NAICS) is 624221 Temporary Shelters with a small Business Size Standard of $12 Million. Contract award will be based upon the best value to the government in accordance with Federal Acquisition Regulation (FAR) 15. This procurement will be an unrestricted solicitation. Purpose and Objectives: The Department of Veteran Affairs (VA) is seeking contractor(s) who can provide emergency, residential housing and case management services to homeless Veterans as part of the Community Based Health Care for Homeless Veterans (HCHV) program. These are not homeless shelters but rather transitional housing programs for VA health care eligible Veterans who require safe and stable living arrangements while they seek permanent housing. The contractor works in close collaboration with VA to provide Veterans community-based, residential environments with sufficient therapeutic services to meet the needs of those Veterans. Contracted Emergency Residential Services (CERS) programs target and prioritize homeless Veterans transitioning from literal street homelessness, Veterans being discharged from institutions, including those in need of medical respite, and Veterans who have recently become homeless. CERS programs, either directly or through linkage with community and other VA services, provide low demand, time-limited services such as supporting mental health stabilization, Substance Use Disorder (SUD) treatment services, enhancement of independent living skills, vocational training, and employment services. Emphasis is placed on referral and placement into permanent housing or longer-term residential programs utilizing VA and/or community resources. Lengths of stay in CERS typically range from 90 to 180 days with the option to extend based on clinical need. The purpose of this sources sought notice is to obtain information from qualified and experienced contractors who could potentially provide housing, case management services, and rehabilitative services as explained above. VA is only seeking information from contractors with current, active residential facilities in upstate New York. Facilities must be in centralized locations, near public transportation, and near areas that provide employment. Before responding, please carefully read and consider the following: As this is a service acquisition, the Small Business Administration and the Federal Acquisition Regulation have a Limitations on Subcontracting requirement. For all small businesses, to include SDVOSB and VOSB, at least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern.
