The VHA, VISN 2, VA Finger Lakes Healthcare Systems requires a contractor to provide medical gases for each of the three (3) VA medical center facilities, their respective clinics, and off-site locations located in their region. The Contractor shall be responsible for providing, filling, transporting, handling, servicing, maintaining, delivering, picking up and storing medical gases and cylinders for all facilities located within VISN 2, VA Finger Lakes Healthcare System. All equipment and materials required to perform on the contract shall be provided by the Contractor (other than government-furnished property). Locations: Canandaigua VA Medical Center 400 Fort Hill Road Canandaigua, NY 14424 Rochester Outpatient Clinic (ROPC) 260 Calkins Road Rochester, NY 14620 Possibly in the future Clinton Crossing 919 Westfall Road Rochester NY 14618 All Canisters owned by Haun Specialty gases except Size D tanks in Canandaigua

Estimated annual usage is 300 refills annually. Twice weekly delivery Size D-Tanks Oxygen 15 Cubic Foot-Leased tanks, Qty 18, Bldg. 8 rm. 3. Estimated annual usage 30 refills. Twice weekly delivery Size B Tank 6 Cubic Foot-Leased tanks Oxygen, Qty 8, Bldg. 8 rm. 3. Estimated annual usage is 20 refills. Twice weekly delivery.
