STATEMENT OF WORK FOR Installation of Concrete Slab and Sunshade Brooklyn Veterans Affairs Medical Center SL-23-05-BK Introduction and Background The VA New York Harbor Healthcare System provides quality health care to veterans using the abilities of all employees supported by our commitment to education and research. The VA NYHHS Engineering Service function is dedicated to optimizing the health care and overall physical environment of the Brooklyn Campus and to ensure the seamless operation of the Medical Center. This typically includes maintenance of structures, grounds, and associated equipment, operations of heating and air conditioning plants, water and sewerage treatment plants, and all associated repairs. The Engineering Service works with Administration and Medical Services in identifying and correcting issues associated with building supervision, construction, modernization, maintenance of utility systems, and associated equipment to assure compliance with regulations, operating standards, and ability to meet the needs of the Medical Service. Major functions of the Engineering Department include functional space requirements (e.g., patient flow, workflow, lines of sight, patient isolation, etc.), smoke control, air conditioning and humidity control, life support systems (e.g., air, oxygen, vacuum, emergency power, etc.), electrical safety, energy conservation, and budgetary constraints. One of the major VA NYHHS Engineering Service s objectives is the prevention of Engineering related and Construction/Maintenance related Safety Hazards. The VA NYHHS Engineering Service is assuring the continuous operation of essential patient services to our Veterans and rapid response to conditions which may interfere with such services. The Brooklyn VA Medical Center is in need of a concrete slab and sunshade canopy structure to be installed in association for the VA s Whole Health Initiative. The location of this concrete slab and sunshade canopy structure is adjacent to Building #2 (Domiciliary Building). Objective Provide all necessary labor and material to install a concrete slab and sunshade canopy structure adjacent to Building #2 (Domiciliary Building) at the New York Harbor Healthcare System Brooklyn Campus. The Contractor shall provide all labor, material tools, equipment, supervision, and other related items required to complete the project as per scope of work and specifications. Contractors are advised to visit the site, verify the existing site conditions to develop their proposal. All expenses towards mobilization at site and demobilization including bringing in equipment, workforce, and materials, dismantling the equipment, clearing the site etc. shall be deemed to be included in the rates quoted by the contractor against various items of schedule of rates and no separate payment on such expenses shall be entertained. The Contractor shall prepare the surface in order to install a concrete slab in association with the installation of a sunshade canopy structure, as necessary, including excavating and compacting the surface. Once the surface is prepared, the Contractor shall perform the necessary work to install the concrete slab, including associated gravel and wire mesh installation work. The contractor shall also purchase and install the sunshade canopy structure. The work shall also include, but is not limited to, restoring all excavated or disturbed areas. The Contractor shall furnish all labor, materials, tools, supervision, and equipment, including safety, necessary to perform all the repair work. The Contractor shall be responsible for the proper and safe removal and disposal of all debris. The Contractor shall also take all necessary precautions and safety measure to protect all pedestrians and vehicles from all construction related activities. Specific Requirements Contractor to ensure the surface to be prepared is free of any large objects. Surface shall be cleared if any objects are present. Contractor to ensure the underground surface is free from any utility lines, conduits, etc. Contractor shall use an appropriate underground surface surveying method to ensure this requirement is met. Contractor shall excavate the surface as per the drawings, including the removal of tree FboNotices. Contractor shall install a filter fabric with a 2 layer of gravel. Contractor shall compact the soil surface and gravel. Contractor shall prepare and form the surface. Contractor shall install a layer of wire mesh. Contractor shall pour and install a 6 D concrete slab. This slab shall be 40 -0 in diameter and has an associated 5 -0 W x 60 -0 L ADA accessible wheelchair ramp. Contractor shall properly backfill any open excavation areas. Contractor shall backfill the soil to restore any disturbed areas. Contractor must have OSHA training, and specialized OSHA training. Contractor shall follow all Infection Control measures and Interim Life Safety measures during construction. All work performed will be in an operational healthcare facility. All work shall be done in compliance with all VA Policy and requirements. All work shall be done in accordance with NFPA, Life Safety, and VA Standards and Regulations, and OSHA regulations. Contractor shall give at least one year on all workmanship and provide warranties. It is the responsibility of the contractor to verify all quantities, field conditions and measurements prior to starting work. The contractor shall notify the CO/CORs and VA Engineering Team of any differing site conditions prior to starting. The Contractor shall provide an English-Speaking working supervisor on-site at all times when work is being performed and for the duration of this project. Supervisor must have a minimum of five (5) years of experience as a supervisor on five (5) projects like this. The contractor shall provide his own storage for materials. The VA is not responsible for loss of tools, materials, equipment, etc. The Contractor shall post visible required warning signs and barriers prior to beginning any work. The Contractor is responsible for any damage caused by his employees. All damages shall be repaired and/or replaced at no additional cost to the government. Contractor shall submit schedule to VA for approval. The Contractor shall not perform any work on utility services without prior coordination with CORs and VA Engineering Team. The contractor shall not interfere in any way with Hospital operations. The contractor shall perform cleanup at any time when work area becomes cluttered and/or restricts traffic. Nevertheless, work area must be cleaned at the end of each work shift. In the performance of all work, the workers must be incompliance with safety regulations governing worker safety (OSHA: 29 CFR 1926 Safety and Health Regulations for Construction; NFPA, Life Safety Codes, etc.). Contractor must coordinate and schedule all work with the VA. Contractor is allotted a period of performance of 30 calendar days. General Requirements 1. The contractor shall be required to have VA issued identification badges for all of his/her personnel working on VA property. Anyone observed without VA identification will be asked to leave VA property. No VA identification, no work, no exception. 2. There will be no smoking at this facility except in the smoking shelter, located in the rear of the facility. No exception. Those caught violating this requirement will be required to leave the facility and/or be escorted off the facility by Police. 3. The contractor is responsible for obtaining his/her own vehicle parking. 4. Vendor personnel must complete VA privacy and information security awareness training . This is a mandatory training for all VA and non-VA employees working at this facility. This training must be completed annually. Completion certification must be provided for each employee that will be onsite, if applicable. 5. The contractor shall ensure that his/her personnel conduct themselves in a professional manner, i.e., no profanity; proper attire; no discourteous behavior to VA employees, etc. 6. In the performance of all work, the workers must be incompliance with safety regulations governing worker safety (OSHA: 29 CFR 1926 Safety and Health Regulations for Construction; NFPA, Life Safety Codes, etc.). The Contracting Officer or his/her designee will notify the contractor of any noncompliance with the foregoing provisions and the action to be taken. The Contractor shall, after receipt of such notice, immediately correct the conditions to which attention has been directed. Such notice, when served on the Contractor or his/her representative at the site of the work, shall be deemed sufficient for the purpose aforesaid. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all and any part of the work and hold the Contractor in default. Safety Contractor is responsible and shall continue management and implementation of a safety and health program throughout duration of the project. The Contracting Officer and VA staff reserve the right to suspend work when patient, staff and contractor's safety and health program is inadequate or non-complying. Contractor shall provide all Personal Protective Equipment for the workers as per the requirement of the site. Work will be stopped in case the proper protection equipment is not found with the workers and the lapse of time shall be at the Contractor s expense. Place of Performance / Delivery It is the contractor s responsibility to accept all deliveries related to the project and coordinate with the CORs.
