The purpose of this modification is to extend the Bid Opening Date. The Bid Opening Date has been extended from July 15, 2022, to Thursday, July 21, 2022, at 2 P.M. via Microsoft Teams. This amendment also revises the Scope of Work to remove the Asbestos Abatement requirement. Please see below revised Scope of Work (SOW). Statement of Work (Revised) Project Title: Kitchen Room Service Project Number: 630-23-106 Description. The Department of Veterans Affairs NYHHS - Manhattan Campus Nutrition Food Service is moving forward with the VA Nationwide Room Service Meal Initiative to increase patient satisfaction. This project seeks to retrofit and prepare the selected kitchen area to install new equipment. This project involves Architectural, Mechanical electrical - plumbing (MEP) roughing to feed the new kitchen equipment and meet code compliance. Prospective Contractor shall provide all material, labor, and equipment to install and commission new kitchen equipment. All work shall be completed by the Go Live Milestone of December 2022. The selected Contractor shall provide Stamped As built Drawing per discipline by PE or License Plumber, License Fire Suppression, License Electrician, License Fire Alarm. Drawings shall be provided for reference only. Contractor shall be responsible to verify site conditions. As-built drawing and submittals shall be submitted for VA approval prior to commence work.. All work must be executed by licensed plumber and electrician and per VA Til site; https://www.cfm.va.gov/til/. Electrical Provide all electrical feed for selected equipment per manufacture recommendation (Attatchement 2) Electrical Devices shall meet VA specifications requirements Remove abandoned electrical devices at kitchen hood area Core drill floor /slab to rough in all conduit from 2FL up. Power feeders PCQ, ERP3E, PCT, LCT, RP3E-1, RP3, ERP3E-3, ERP3E-2, ERP3E-1 panels. Waterproof boxes are required. Coordinate with equipment supplier/installer (Attachment 2) Rough in new Fan Coil Unit (4) per manufacture specs coordinate with mechanical contractor Provide power for abatement contractor Plumbing Furnish and Install DCWS-R and DHWS-R for (2) Sinks (Attatchement 2) Furnish and Install water line for coffee machine per manufacture requirement Furnish and Install Gas Line to feed burner and grill per code, coordinate with equipment installer Furnish and Install Fire Suppression System for burner and grill per FDNY and NFPA code Furnish and Install (2) Water Filters per manufacture requirement (Attachment 2) Core drill floor /slab to rough in all piping from 2FL up Demolish abandoned devices Provide condensate drain for existing walking Cooler Cap to main abandoned lines Pipe Insulation Coordinate with equipment supplier/installer to meet requirement Provide water to abatement contractor HVAC Upgrade Hood Exhaust Ducts (12) per NFPA/FDNY Code Provide quarterly Filter /Duct clean up Test, Adjust and balance new and existing kitchen HVAC Replace of existing heating device (7 total), steam piping connection, traps, valves and thermostats and all associated appurtenances. Furnish and Install (6) New heating system with all new associated appurtenances. Demolish exiting Induction Unit (4) and PTAC Units (2) and all associated appurtenances. Furnish and Install new PTAC Units (2) total 12KBTU) with all new associated appurtenances. Furnish and Install new FCU (2) total 12KBTU) with all new associated appurtenances and further BMS control. General Contractor GC to coordinate (trades) daily activities Patch to match existing wall and ceiling Patch area impacted by the removal of the Induction Units Open and Close ceiling below at 2Floor level to allow plumbing and electrical rough in. Install Safety barriers during active work at all times. Fire stopping all wall/slab penetration Paint areas impacted by new works including offices. Scrup and wax 3000 SF Kitchen Floor (potential phasing) Protect floor, wall, medical equipment ICRA barrier required at all times Asbestos Abatement Removal of Asbestos (pipe insulation) 100 LF Glovebag Asbestos Abatement Removal of Asbestos 200 SF Third Party Inspection (monitor) REMOVED Fire Alarm Tie in suppression system to main FA System include graphic to update computer background graphics. Asbestos Abatement REMOVED The government will order selected equipment for this project. Contractor shall verify and coordinate with equipment supplier for final tie in. Contractor shall furnish and install the accurate feeds for the selected equipment. Period of Performance The Period of Performance (PoP) shall be120 Calendar Days from issuance of Notice to Proceed (NTP). Work Hours All work will be executed Off Hours between 7 PM to 4 AM. Work could be schedule during normal business hours (7 AM to 6 PM), but must be requested and approved by the Contracting Officer. Reference Documents All Carpentry, Electrical, Mechanical and Plumbing work shall adhere to VA Technical Specifications. Refer to VA Specifications General Condition Construction Quality Assurance/Quality Control Contractor to provide full time Quality Assurance onsite for daily monitoring of quality of work. Contractor to provide Construction Quality Control Report containing preparatory inspection, initial inspection, and follow-up inspection. Construction Safety Program Contractor to provide OSHA Certified Competent Person (CP) Safety Inspector to ensure that the specific safety requirements for construction operations are implemented during facility projects. Provide daily monitoring and written report of compliance with safety regulations for construction site. Provide daily reports of safety inspections (include safety violations and corrective actions taken). Participates in the VHA facility multidisciplinary team established for the construction safety committee. Contractor to fill out and submit ICRA form (Healthcare System Policy No: 138-27) and ILSM form (Healthcare System Policy No: 138-13) for review. These forms must indicate VA required measures to be implemented by the Contractor. Upon approval by the Engineer, forms will be signed as indicated on the forms. These forms must be posted at each worksite by the Contractor. The Contractor shall be responsible to see that these forms are updated as required. Temporary Parking ONLY to Unload materials Contractor shall procure offsite parking for their vehicles. Secure offsite parking shall be within ¼ mile of the site, shall be accessible 24 hours per day / 7 days per week for the duration of the project. Contractor shall be responsible for all associated parking fees until completion of the project. ID Requirements 1.) (1) Government or State issued valid ID s. 2) VA form 0711 3.) TB test. 4.) OSHA training 30. COVID Safety Protocols The E.O. requires agencies to include a clause in applicable contracts requiring contractors and subcontractors at any tier to comply with all guidance for contractor workplace locations as published by the Safer Federal Workforce Task Force (Task Force Guidance). The clause applies to solicitations and contracts for services, including construction. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Debris Removal All construction debris will be removed daily. No debris will be allowed to remain overnight or on the weekend. The freight elevators will be used to remove all debris. The hours debris can be removed after 5 PM (17:00 hours) to 5 AM (05:00 hours). The passenger elevators will not be used to remove any debris. Contractor shall comply with VA Privacy Acts and training requirements. See attachment A.
