To be eligible for consideration, ambulette qualified firms capable of providing assisted transportation (taxi, wheelchair van or littler/stretcher vehicle) service for the beneficiaries of the VA NYHHCS, will be located at the following campuses: Manhattan Campus Brooklyn Campus St Albans Community Living Center 423 East 23rd Street 800 Poly Place 179-00 Linden Boulevard New York, NY 10010 Brooklyn, NY 11209 St Albans, NY 11425 Community Outpatient Clinics (CBOC) Harlem Community Clinic 55 West 125th Street New York, NY 10027 Staten Island Community Clinic Queens Community Clinic 1150 South Ave. Thomas P. Noonan VA Community Clinic 3rd Floor 47-01 Queens Blvd Staten Island, NY 10314 Sunnyside, NY 11104 Yonkers Community Clinic White Plains Community Clinic 124 New Main St. 23 South Broadway Yonkers, NY 10701 White Plains, NY 10601 Offerors will have been engaged in offering this type of service and scope no less than two years and be capable of presenting certification of such with verifiable references. The applicable North American Industry Classification System (NAICS) code assigned is 485991. There is no solicitation at this time. This request for capability information does not constitute a request for proposals; submission of any information in response to this market survey is purely voluntary; the Government assumes no financial responsibility for any costs incurred.
