Page 3 of 3 THIS IS A SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS The Department of Veterans Affairs is conducting a MARKET RESEARCH SURVEY to obtain information regarding: (1) the availability and capability of qualified large business sources (2) the availability and capability of qualified small business sources; (3) whether they are Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concern, (4) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition, and (6) current market price. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. The NAICS code is 339113 Description of The Requirement: Bard Peripheral Vascular Luminexx Biliary and Lifestent Stents in various quantities and sizes for Consignment Agreement The Syracuse VA Medical Center has a requirement for various sizes and quantities of Bard Luminexx Biliary and Lifestent Stents to be supplied to the facility under consignment agreement terms. The vendor would be responsible for maintaining and stocking the inventory, without charge to the VA. Payment would be issued for items used from the inventory only. Interested vendors must be authorized Bard distributors. No gray-market items will be allowed within the facility. Service Location: Syracuse VA Medical Center: 800 Irving Avenue, Syracuse, NY 13210 POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large, VOSB, SDVOSB), FSS/NAC/GSA contract number (if applicable), DUNS Number and approximate pricing for the above mention stent types.
