This is not a solicitation for bids, proposals, proposal abstracts, or quotations. The purpose of this Sources Sought Notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. The responses received from interested contractors will assist the Government in determining the appropriate acquisition method. The Department of Veterans Affairs Network Contracting Office, (NCO) 2 is in search of SDVOSB/VOSB organizations that are capable to provide repair services for two (2) Clark C15L forklifts for Northport VAMC listed in the below Statement of Work. STATEMENT OF WORK Background: The Northport VAMC requires repair services for two (2) Clark C15L forklifts used in the warehouse for a large number of items in inventory. The forklifts are essential for loading and unloading pallets and balanced materials, as well as removing and transporting items from delivery vehicles. Scope: The contractor shall provide the following services: Clark C15L Forklift (ID: P15211033977OHF) Full brake job which shall include removal and replacement of brake shoes, mater cylinder, brake cables, wheels cylinders, seal and hardware. Overall inspection of brake system; adjustments made as needed. Installation of new muffler including pipes, hoses and clamps. Transportation of forklift to contractor s place of business for repairs. Transportation of forklift to Northport VAMC after repair. Clark C15L Forklift (ID: P15211033977OKF) Remove and install new oil pressure switch. Requirements: Repairs shall be completed by a qualified mechanic or certified operator. After repairs are completed, the contractor shall perform an overall courtesy inspection of the forklift s engine, steering, and hydraulic systems to ensure proper operation as well as look for red flags such as cracks or bends in the fork assembly, leaks, worn tires or signs of damage or excessive wear, and report any issues to the point of contract at the Northport VAMC. No additional work or repairs shall be completed without prior approval from the Northport VAMC. The contractor shall use only new OEM (Original Equipment Manufacturer) parts or new parts that comply with OEM standards proposed by the product manufacturers. Contractor shall guarantee all parts and labor for a period of 90 days from the date of service. The Northport VAMC guarantees that equipment shall only be used in accordance with manufacturer guidelines since the original repair and that no other repairs have been attempted on the forklift(s) within the warranty period. If the manufacturer of parts has a longer warranty period, the contractor shall extend that warranty to the Northport VAMC. Other: This contract does not involve VA sensitive information/data. Capability Statement Requirements: RESPONSES MUST INCLUDE THE FOLLOWING INFORMATION: Company Name, Address, Size Standard, DUNS #, Cage Code Point of Contact name, phone number, and email address NAICS Code 811310 and Small Business Size Standard $8.0 million MUST be assigned to company Capability Statement: Provide proof of past performance (provide 2 Clark C15L forklifts repair service contracts you have worked on in the last 5 years or of the company that will be performing the service) Specify the specific line items in which you intend to provide. If you will NOT be performing the C15L forklifts repair services, you must provide a Letter of Intent from the business you intend to use (explaining their agreement with you to perform the services according to the solicitation), along with references of their past C15L forklifts repair services. *NOTE Acquisitions will only be set aside for SDVOSBs/VOSBs if two or more responses are received with the required information listed above, by the due date specified below. Only SDVOSB and VOSB offerors are invited to respond to this Sources Sought Notice via email to Eulanda.James@va.gov, along with any other questions or concerns, no later than 12: Noon Eastern Standard Time (EST) 02/25/2020.
