POC: Dayna Dickson at dayna.dickson@va.gov DESCRIPTION: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number 36C24219Q0401 is issued as request for quotation (RFQ). This solicitation is being solicited as lowest price technically acceptable (LPTA). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-98. The NAICS Code for this requirement is 339113 Surgical Appliance and Supplies Manufacturing and the size standard is 1000 employees. To be eligible for award vendors must be registered against NAICS Code 339113. Line items and quantities are as follows (items or equivalent items): -------------------------------------------------------------------------------- Active Treat Bleeding Control Kits QTY: 700 Each kit should contain: One compression wrap tourniquet. One compression bandage for large wounds to control severe bleeding. One compressed gauze to pack a severe wound or to wrap wounds. One hypothermia blanket to wrap around a victim to keep them warm and to help manage shock. One safety cutter to remove clothing, seat belts, or debris from a victim or their injury. One pair of nitrile gloves size large. All items much have minimum of a 5-year expiration date. All kits must be preassembled. Items do not have to be sterile. Kit dimensions should be no bigger than 7 3/8 x 11.5 x 1.1 and weigh no more than 12 oz. Kit itself should be a zipper or snap closure nylon bag that has a handle on the outside so the kit can be hung up. Installation: FOB Destination. Education and Training Requirements: The Vendor shall be responsible for ensuring that vendor employees providing work on the required equipment are fully trained and completely competent to perform the required work. Recalls: The Vendor shall immediately notify all affected VA Medical Centers and designated POC in the event of a device recall, removal, required adjustment, or required modification that is suggested or mandated by the Vendor, distributor, manufacturer, or any regulatory or government agency.
