The Department of Veterans Affairs intends to negotiate with OEM, SB, MIND MEDIA USA INC, to procure the NeXus 10 MKII Biofeedback Unit for distribution to 11 Headache Centers of Excellence (HCoE) sites within the Department of Veterans Affairs (VA), located at the VAMC West Haven, CT. OEM, SB, MIND MEDIA USA INC is the only company that can provide the NeXus 10 MKII Biofeedback Unit; a sole source letter has been obtained. Since the intent is to procure this on a sole source basis, no solicitation will be posted to Contracting Opportunities SAM.gov. The authority for this action is FAR 6.302.1-1 Only One Responsible source and no other supplies or services will satisfy agency requirements.  The NAICS 334510. The size standard is 1,250 employees. Statement of Work (SOW) A. GENERAL INFORMATION The requirement to procure Nexus Mark 10 biofeedback units for distribution to 11 Headache Centers of Excellence (HCoE) sites within the Department of Veterans Affairs (VA). The HCoE program delivers specialized, multidisciplinary care to veterans suffering from chronic headaches, including migraine and post-traumatic headache, and emphasizes evidence-based, non-pharmacological treatment modalities. The Nexus Mark 10, a multi-modality biofeedback system, will support clinicians in providing personalized, real-time physiological feedback to veterans, enhancing self-regulation strategies and symptom management. This acquisition supports VA s ongoing mission to provide innovative, holistic, and patient-centered care, and will expand access to integrated treatment tools across multiple HCoE locations nationwide. B. GENERAL REQUIREMENTS Service Period: N/A. Purchase of biofeedback units C. Confidentiality Plan and Data Security Procedures Contractor personnel will not have access to secure or sensitive data. The service under this contract is not used for collection or storage of personal or confidential data of Veterans or non-veterans. E. INSPECTION AND ACCEPTANCE CRITERIA Final inspection and acceptance of all work performed, reports and deliverables will occur at time of service F. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the VA s Contracting Officer (CO). A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. G. SECURITY No patient data regarding Veteran nor non-veteran will be used in this service. H. CONTRACT TERMINATION VA has the right to terminate (in whole or in part) this order at any time in accordance with the termination clauses of the governing contract. The Contractor will be paid only for the services rendered up to the point of receiving the termination notice, and then only to the extent that those services meet the requirements of this SOW.
