This is not a request for proposal, please DO NOT submit any Cost or Pricing information with your Firm s Statement of Interest response. SOURCES SOUGHT: The VA Providence Healthcare System (VAPHS), 830 Chalkstone Avenue, Providence, RI 02908, has a requirement for a Firm Fixed Price (FFP) Contract, for Professional Commissioning Services (for an upcoming Elevator Upgrade and Modernization Project). COMMISSIONING SCOPE OF WORK: The objective of the commissioning process is to ensure that the elevator system and its interface with other systems (fire alarm, etc.) are designed, installed, tested, and operates in accordance with the project specifications and design documents. Services shall include items, and be provided in accordance with the VA Master Specifications related to commissioning referenced herein, the entire Building Commissioning Process Manual is available on the VA Technical Information Library at http://www.cfm.va.gov/til/ Commissioning services will be for an upcoming elevator upgrades and modernization project and will include the nine (9) elevators numbered P2, P3, P4, P7, P8, S1, S5, and S6 (5 passenger and 3 service) located in the main hospital Building 1 and one (1) elevator numbered P11 located in Building 35 (1 passenger). It is the intent of the VAPHS-FMS to obtain commissioning services for the design, construction, and warranty phases of an upcoming elevator upgrade and modernization project. NAICS: The NAICS code for this Project is 541350 Building Inspection Services. Small Business size standard is $11.5 million. MAGNITUDE OF CONSTRUCTION: is between $25,000 and $100,000. STATEMENT OF INTEREST: Interested and qualified contractors must indicate their interest by submitting a brief written 1-3 page Statement of Interest (to include the below items A D): Submit a statement of interest that your company intends to submit a proposal when the VA submits a RFQ for this commissioning project and include your Firm s SAM UEI. The statement of interest must provide clear and specific information that describes your company s commission of elevator upgrades experience, per the attached Scope of Work. Statement of interest can be up to 3 pages. Statement of interest can also provide a brief description of up to 3 contracts and/or Task Orders similar in size, scope, and complexity as the commissioning requirements described in the attached Scope of Work. Interested company s must be registered in the System for Award Management (SAM). Interested company s can register at https://www.sam.gov/content/home. SDVOSB and VOSB firms must be verified by the Center for Veteran Enterprises (CVE) under NAICS 541350.
