The Veterans Health Administration, Network Contracting Office One (NCO 1) intends to release solicitation 36C24122Q0380 for Acute Life Support (ALS), Basic Life Support (BLS) Ambulance Transportation Services for beneficiaries of Connecticut VA Medical Center. Services will include the provision of ALS, and BLS Ambulance transportation to beneficiaries Connecticut VA Medical Center. The full solicitation will be posted on the System for Award Management (SAM) located at https://beta.sam.gov on or about January 3, 2023. Performance period will be 365 calendar days after award, with four 1-year option periods. This is a pre-solicitation notice synopsizing an upcoming request for quote (RFQ) solicitation to satisfy the requirements of FAR 5.201 and 5.203. All questions should be directed to the Contract Specialist, who will consolidate all questions and release a FAQ amendment (if necessary) once the solicitation is released. All interested parties should register via the SAM portal at https://sam.gov/SAM to be eligible for participation in the solicitation. It is the responsibility of the offeror to monitor and download amendments from SAM which may be issued to this solicitation. Prospective firms must be registered in the system for award management (www.sam.gov) and in the Vendor Information Pages (VIP) database on Vet Biz (www.vetbiz.gov) if claiming Veteran preference prior to award.

The current primary NAICS code of this requirement is 621910 (Ambulance Services), which has a size standard of 16.5 million. 4. SUBMITTAL INFORMATION: Contractors having the skills and capabilities necessary to perform the stated requirements should submit a response as detailed below. Companies should provide clear and unambiguous evidence to substantiate their capability to fulfill the requirements above. If this information is not provided, your firm cannot be considered a viable source for this procurement. Interested parties shall furnish the following minimum information: Company name and address ensure your company is registered in the System for Award Management (SAM) at www.sam.gov. Dunn and Bradstreet (DUNS) number. Provide SAM unique entity identifier (UEI) (if available). Company point of contact name, telephone number, and email address. Evidence that you are a small business (if applicable): Small Business ensure you are listed as small business in SAM, reps and certifications. Provide the NAICS codes you are listed as a small business under. CVE certified SDVOSB or VOSB (or in the process of certification) ensure you are certified in the CVE veteran database at www.vetbiz.va.gov/. Provide evidence of certification. If your company holds a Federal Supply Schedule (FSS) Contract, list the GSA Contract Number and SINs. Capabilities/Qualifications: A written response providing clear and unambiguous evidence to substantiate the capacity to fulfill this requirement. Description of the capabilities/qualifications/skills/required licenses your company possesses to perform services in the geographic areas described above. Evidence of existing capability to perform the required transportation services with all necessary resources in place upon the execution of the awarded contract. 6. Past Experience: Brief summary of the company s history relative to similar requirements in size and scope. Provide the information on projects of similar size and scope for which the responder was a prime or subcontractor including size of fleet and numbers of employees.
