DEPARTMENT OF VETERAN AFFAIRS STATEMENT OF WORK PATIENT LIFTS Preventative Maintenance/Inspection and Emergency Services 1.0 INTRODUCTION Manchester VAMC has a requirement to provide preventative maintenance and inspection services as well to provide emergency repair services for our patient ceiling lifts. This work is to be conducted by a factory authorized vendor. This scope of work is to provide these services for the specific patient lift equipment at VAMC Manchester. 2.0 BACKGROUND The Medical Center currently has 78 ARJO patient lifts in use, in various clinical areas throughout the facilities onsite and offsite locations at the CBOCs (Conway, Tilton, Portsmouth, & Somersworth). These lifts require routine annual inspections and preventative maintenance three times a year in accordance with manufacturer' as well as periodic emergency service to ensure compliance with Medical Center policies and the Joint Commission. 3.0 SCOPE The vendor shall provide preventive maintenance and inspection as well as emergency repair services. The vendor shall provide on call services to include labor costs. The vendor will be able to respond to fix issues within four (4) hours. The vendor shall supply minor parts and materials (<$200 per lift per year) as needed for necessary upgrades, replacement, and repairs. It is anticipated that some lifts will be added and removed during the contract period. The vendor shall price their maintenance based off the 78 ARJO patient lift number and shall provide a cost per additional lift beyond that if the VA Medical Center has a need to increase the total number in use. 4.0 APPLICABLE DIRECTIVES MCP 00-24 Safe Patient Handling MCP 138-18 Non-Medical Equipment Management Program 5.0 PERFORMANCE REQUIREMENTS Vendor shall complete and submit a Corrective and Preventative Maintenance Checklist for Ceiling Mounted & Mobility Lifts document for government review prior for each patient lift that is to be inspected or repaired prior to work start. The vendor shall allow the facility to review and recommend changes and additions to the checklist prior to work being performed. A Corrective and Preventative Maintenance Checklist shall be provided for each lift inspected. The vendor shall submit the inspection report and preventative maintenance checklists to the facility via email within 48 hours after work completion. The vendor shall submit technician certification/licensure prior to work start. Copies of these are also to be submitted with the completed checklists. Vendor shall submit all necessary safety plans to include, but not limited to; equipment specific Lock-Out Tag Out procedures, Electrical Safety, plan and SDS for any chemicals used to the COR for approval prior to working at the Medical Center. Vendor shall schedule all site visits, to include emergencies, with the COR. Vendor shall furnish authentic parts from the equipment manufacturer. The determination of when to replace parts is at the recommendation of the technician, with the COR s concurrence. Additional service charges may be incurred to install parts during scheduled inspections or callbacks as specified in this agreement. Vendor shall operate on the premises during the hours of 0700 and 1630, unless otherwise authorized by the COR. Vendor shall respond within 4 hours for all emergency call backs. Work sites shall be left in the same manner of cleanliness and order in which they were found. Vendor will provide their own waste disposal, if any. All vendor technicians performing work on site must report to Facility Service in Building 6 to the contractor badge check in/out room to sign in on the contractor log and get a Contractor badge. Badges must be returned, and log signed out prior to departing the premises. Vendor shall then coordinate with the COR prior to performing their work. Vendor shall perform preventative maintenance and inspection as well as emergency maintenance on all patient lifts in accordance with manufacturer s specifications. Annual inspection/preventative maintenance shall meet the requirements of the equipment manufacturer. As a minimum, this shall include but not be limited to the following list of tasks: Check for visible damage to the lift surfaces, finish, etc. Check to ensure that bolts are tightened on the carriage. Check the emergency stop function. Check the hand control functions. Check the electrical emergency lowering and raising functions. Check the function of the squeeze protection limit switch. Check the lift strap for visible damage. Inspect the sling bar bearings and safety latches. Check the charger function. Check the type/model identification decal. On ceiling lifts: Verify attachment to overhead structure is intact. Attachment points shall not show signs of missing or loose nuts or bolts. Tie rods shall not be deformed. Attachment hardware shall not be attached to any other equipment above the suspended ceiling. No other equipment shall be hung from the ceiling lift or its support hardware. The vendor shall notify the facility within two (2) hours on any lifts that is deemed Out of Service due to failed inspection, damages, and pending parts/materials. The vendor is to immediately mark the equipment out of service to include lock out tag out if needed. The vendor shall provide routine manufacture in accordance with manufacturer s specifications to include preventive maintenance 3 times a year on patient lifts. The vendor shall provide service labor, materials, and parts necessary to support as needed and emergency calls for repairs and replacements. If cost exceeds the agreed upon limitation (>$200 per lift per year), then a list of repairs required as a result of the inspection will be provided with a cost proposal for the repair within 24 hours to the COR. Personnel must wear the proper personal protective equipment (PPE) at all times to include a surgical mask. Work done under this project must comply with infection control requirements. The contractor shall receive all deliveries of materials, equipment, tools, or other goods off of station, and bring them to the job site. The Manchester VA shall not accept deliveries for project purposes. Special care will be taken to secure tools and construction equipment. 7.0 Background Information Patient Lift Inventory (Attached)
