Amended quotes will be accepted.This extension is in recognition of the impact of COVID-19 shortages on the ability to provide adequate samples for evaluation. Amendment is also made to the attachment RFQ to match the posting SDVOSB set-aside. Part numbers have also been stricken from the amendment. The following questions are answered regarding the RFQ. 1.Q: In regards to RFQ 36C24121Q0137 Custom Angio Packs, please provide the manufacture name and part number for the 2 Angio Packs listed on the solicitation in order to provide an accurate quote containing the components VAMC West Roxbury is requiring in their packs. A: The prior contract award provided Medline products. Specifically Part Number - DYNJ38120B, and DYNJ38119A, for the Drape Pack and E.P. Device Pack respectively. It is further noted that this solicitation is NOT a brand name solicitation. Accordingly, part numbers have been stricken from the Line Item descriptions under the amended RFQ. Orders are to be placed during standard hours of care Monday through Friday from 0800 (8:00 AM EST) until 1700 hours (5:00 PM EST), not including Federal holidays. Upon the order being placed, the Contractor is to send confirmation of receipt to the ordering official via email. The Contractor will designate a primary and alternate means to place orders i.e. phone, email, fax etc. The Government shall ensure all routine orders for Custom Packs will be placed by VA Medical Center West Roxbury to the Contractor a minimum of 5 business days prior to treatment and before the clinic close of business. The Contractor shall receive orders for the Custom Packs in CLIN 0001 and 0002 from Authorized Ordering Agents assigned to the VA Medical Center West Roxbury. The Contractor shall ensure the Authorized Ordering Agent’s name and contact information that placed the order is included. VA Medical Center West Roxbury will supply a list of Authorized Ordering Agents to Contractor when award is made. Minimum and Maximum order limitations; see Clause 52.216-19 -- Order Limitations contained herein. The Contractor shall notify VA Medical Center West Roxbury ordering official immediately should the inability arise to provide the Custom Packs by the required date and time.
