RENTAL STATEMENT OF WORK The vendor shall provide various Wound V.A.C.s upon the request from this facility. The vendor shall only charge the rate as indicated on the Federal Supply Schedule that was provided from the vendor. The vendor will submit Invoices to the assigned Contracting Officer s Representative (COR) for verification prior to requesting payment. The invoice shall include Patient ID (ex. M1142), Bldg. Number, Ward, and Room Number. Once verified, the vendor will submit for payment via the e-invoice website. Any request for payment not submitted for verification prior will be delayed pending verification. Payment request not listing the above required items, and/or the correct Purchase Order Number will be delayed and/or rejected for correction. Due to COVID-19 all vendor personnel will be required to enter the facility via the Bldg. 2 or 78 entrance to be screened by VA Staff. All New Consignment Wound VACs are to be delivered the facility Urgent Care for storage until needed. All Wound VACs needing to be picked up need to be picked up on the ward in which the patient was assigned. The vendor will only accept orders from, Mr. John McHale (COR), Mr. Glen Hevy, Ms. Pamela Sardy, or Logistics Personnel in Mr. McHale s absence. If not authorized by the staff listed here it could cause possible delay in payment. All billing questions or issues shall be directed to the COR.
