THIS SOURCES SOUGHT/REQUEST FOR INFORMATION IS ISSUED FOR INFORMATION AND PLANNING PURPOSED ONLY AND IS NOT CONSIDERED A SOLICITATION. The purpose of this sources sought/RFI is to gain knowledge of businesses that are capable of fulfilling the following work requirement: Statement of Work Introduction/Background The intent of this request is to send out our water samples to a laboratory that is environmental certified to perform dialysis water cultures, endotoxin testing, and chemical analysis per the Association for the Advancement of Medical Instrumentation (AAMI) guidelines. The quality of dialysis water is critical and poses health risk and potentially death if dialysis patients are dialyzed through poor quality dialysis water. The Dialysis Unit cannot operate if dialysis water does not meet the AAMI guidelines. Scope of Work This solicitation is for service of our dialysis water sample testing. The contractor shall perform AAMI Contaminant panel testing, perform LAL/Endotoxin testing, and perform Colony Count testing (48-hour test). The results will be immediately reported upon completion of testing. Period of Performance One base year plus four option years. The options may or may not be exercised. Place of Performance All work occurs at contractor s location. Water samples will be sent via mail, and all testing is done on site at the contractor s facility. Work Requirements The contractor shall complete the following services: Provide all sample collection supplies defined as kit to include: Shipping box Freezer pack Requisition Test tubes and bottles Shipment of supplies to clients Restock kits and re-send to client after use Kits will be delivered within 5 days of contract award Perform Colony Count Culture-48 Hr. test Perform Endotoxin/LAL Test Perform AAMI Contaminant Panel Perform Gram Stain Testing Provide Shipping of kits/supplies to customer upon award of contract Report results immediately upon completion of testing by email or fax Phone support and E-mail communications to discuss failures or other testing issues Acceptance Criteria The acceptance of all deliverables will reside with the Nurse Manager of the Dialysis Unit. The Nurse Manager will ensure the completeness of the service and ensure the scope of work has been met. Responses to this sources sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses for a solicitation announcement. Responses to this sources sought/RFI are due by September 3, 2020 at 12:00 PM EST time. Responses to this notice shall be submitted via email to issa.shawki@va.gov. Telephone responses shall not be accepted. If a solicitation is issued it shall be announced at a later date, and all interested parties must response to that solicitation announcement separately from the responses to this request for information. Responses to this notice are not a request to be added to a prospective bidders list or to receive a copy of the solicitation. The NAICS code for this requirement is 541380 and a small size standard of $15 Million.
