No need for set-up. 3. Background: We need one additional unit to house mice in the ARF. We also need some replacement Parts/cages for the units we currently have 4. Performance Period: Date of award good until after the PO is created and the items are delivered. 5. Type of Contract: Sole Source (or open market) decided by the Contracting Officer. CONTRACT AWARD MEETING No contract award meeting will be necessary. GENERAL REQUIREMENTS The Contractor will ship the system. The units will be used within the ARF at the JP campus SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Shipment and delivery of items. Items will be delivered to the warehouse E. EVALUATED OPTIONAL FEATURES No need for contractor set-up on site. F. SCHEDULE FOR DELIVERABLES The Contractor will deliver when they have received a PO from the Contracting Officer at the VA G. CHANGES TO THE STATEMENT OF WORK The process for making changes to the SOW is presented in this section. Making changes by any other means may require ratification of an unauthorized action requiring the approval. H. REPORTING REQUIREMENTS The equipment will be received by Diane Ghera, VMU/ARF Supervisor I. TRAVEL N/A J. GOVERNMENT RESPONSIBILITIES No extra space or effort is required from the West Roxbury facility. K. CONTRACTOR EXPERIENCE REQUIREMENTS Qualifications of Key Personnel: The Contractor shall be familiar with all packaging and shipping for safe delivery to Diane Ghera at the Jamaica Plain campus of the VA Boston Healthcare Systems.
