PERFORMANCE WORK STATEMENT Introduction This is a request to have a contract created for our nitrogen, oxygen, carbon dioxide mix, carbon monoxide mix, small and big tanks. To be delivered to CBOCs as well as needed. If contract gets awarded to a well-established company it is requested this contract be a base year plus 4 years. Requirement The cylinder gases to be purchased include, but are not limited to compressed Medical Air USP, Oxygen USP, Carbon Dioxide USP, Nitrogen NF, Nitrous Oxide USP, Helium USP, Liquid Nitrogen and specialty medical gas mixtures. These will be utilized at the Manchester VAMC and CBOCs in Portsmouth, Somersworth, Tilton, and Conway. Cylinders need to be a single-step system. Fiscal requests that billing from the vendor be invoiced in two separate charges one for the product and one for any shipping charges that incur. Scope Estimated dates of the base year: 10/1/19 9/30/20 Need deliveries done weekly prefer Thursdays to not disrepute various departments pickup/drop off schedules. CBOC s will be filled as needed. Liquid nitrogen twice a month two 5 LT canisters to be filled. For deliveries to the main campus the vendor will need to check in to the warehouse and then bring the medical gas tanks to the morgue/gas storage area and to secure them. For CBOCs and the Main Campus empty tanks will need to be removed when new tanks are delivered. Product Specifications All medical gases shall conform to specifications prescribed in the United States Pharmacopeia and the National Formulary. All medical gas manufacturers and fillers of medical gases shall be registered with the FDA; and all medical gases shall be manufactured, processed, packed, transported, and stored according to the FDA s Current Good Manufacturing Practice (CGMP) regulations and Title 21, Code of Federal Regulations, Parts 210, 211 and 201 for labeling. All products, equipment, and services provided under the contracts awarded under this solicitation shall be in compliance with all applicable Federal, state, and local regulations. Valves need to be the same as the current one step turn on and off for proper patient safety. This is a Sources Sought Notice only.
