COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS TIERED EVALUATIONS INCLUDING LARGE BUSINESS CONCERNS This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 in conjunction with Part 13 of Federal Acquisition Regulation (FAR), as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This Solicitation is issued as a Request for Quote (RFQ), and the solicitation number is 36C10X23Q0146. This notice incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2023-01, effective December 30, 2022, and the Veterans Affairs Acquisition Regulations (VAAR). This Solicitation is being conducted as a tiered evaluation: The tiers are defined as follows (See Evaluation Section for details): Tier 1 - Service-Disabled Veteran-Owned Small Business (SDVOSB) Concerns Tier 2 - Veteran-Owned Small Business (VOSB) Concerns Tier 3 - Small Business Concerns Tier 4 - Large Business Concerns The North American Industrial Classification System (NAICS) Code is 621999 All Other Miscellaneous Ambulatory Health Care Services, and the business size standard is $18.0 million. All interested companies shall provide quotations for the following services: See Attachment A Performance Work Statement See Attachment B Government Responses The contract resulting from this solicitation will be a Firm-Fixed Price contract and will include the appropriate clauses for the tier awarded. The anticipated Period of Performance will be a six (6) month base period from July 1, 2023, through December 31, 2023, with four 12-month option periods. Any award resulting from this solicitation will be issued on a Standard Form 1449. The following FAR and VAAR clauses and provisions apply to this procurement:
