Solicitation Number 36C10B21R0017 is issued as a Request for Proposal (RFP) for Local Exchange Carrier (LEC) telecommunication data services at the service delivery points (SDP) as listed in the Performance Work Statement (PWS). The services required are 24-Strand and 2-Strand Dark Fiber Connections. Specific requirements are listed in the PWS (see section B.4). All quotes shall contain the minimum information required under FAR 52.212-1, Instructions to Offerors, Commercial Items to include its associated addendum (see E.5). All proposals received in response to this solicitation will be evaluated in accordance with the section of this document titled, 52.212-2 Evaluation-Commercial Items (Modified) (see E.6). Your proposal, in accordance with FAR 52.212-3 Offeror Representations and Certifications, Commercial Items, shall list your DUNS number, CAGE code, Company name, Point of Contact, and phone number. All offerors are cautioned to pay close attention to the proposal submission instruction in section E.5 and to return the completed attachment found in section D.
