[End of Amendment No. 00005] The purpose of this amendment (4) is to: 1) Extend the Submission due date from May 17, 2021 to June 7, 2021 at 12:00 Noon Eastern Standard Time. 2) Revise the RFP No. 693JK421R50000X (Now numbered to 693JK421R500004)and provide Government responses to the Offerors questions in regards to the RFP as detailed in Attachment #2. This is a revised solicitation for Personal Assistance Services (PAS). There has been significant revision to this RFP and the Statement of Work (SOW). Interested parties are responsible for monitoring this site to ensure they have the most current information in reference to this acquisition. The Department of Transportation (DOT), Office of the Senior Procurement Executive has a requirement for the subject support services for its Office of Disability Resource Center (DRC). The DOT anticipates the award of a Firm Fixed Price contract from this revised RFP. The contract award shall consist of five years, one (1) base period and four (4) one year option periods. Award will be based on the best value. Interested Offerors are requested to provide the Technical and Price proposals in accordance with Section L in response to Section C of the revised RFP. The Contractor will ensure provision of personal assistance services to DOT and U.S. Coast Guard (USCG) civilian agency employees, including but not limited to those listed in Specific Work Requirements in the RFP, to employees identified by DRC staff.
