1. Introduction/Purpose The Federal Aviation Administration (FAA) is conducting a Market Survey, requesting capability statements from vendors operating an on-call laboratory capable of providing industrial hygiene and environmental analysis as requested. Interested entities must be able to conduct analysis of formaldehyde, lead, silica, asbestos, and additional analytes as specified below. Vendors must specify which tests they are capable of performing as the prime vendor, and indicate which tests will be performed by a third party. 2. Interested entities are reminded that – a) This is not a Screening Information Request (SIR) or Request for Proposals of any kind; b) The FAA is not seeking or accepting unsolicited proposals; c) The FAA will not pay for any information received or costs incurred in preparing the response to this submission request; and d) Any costs associated with this request is solely at the interested entity's expense. 3. North American Industry Classification System (NAICS) Code applicable for this requirement is 541380 – Testing Laboratories. 4. Submission and Qualifying Requirements Interested entities are requested to submit capability statements (not to exceed three pages) that demonstrate they are capable of performing the following tests adhering to the following qualifications: a) Interested parties must be capable of accepting sample submissions 24 hours/ 7days a week and providing expedited turnaround of the results in as little as 3 hours if requested. b) Interested parties must be registered in the System for Award Management under NAICS code 541380. c) Interested parties must demonstrate their participation in the National Voluntary Laboratory Accreditation Program (NVLAP) as recognized by the American Industrial Hygiene Association (AIHA). d) Interested parties must conduct all analyses in accordance with methods approved by the National Institute for Occupational Safety and Health (NIOSH), the Occupational Safety and Health Administration (OSHA), or the Environmental Protection Agency (EPA), unless otherwise authorized. e) Interested parties must be capable of providing the following bulk or air analysis: •Asbestos Bulk Samples (PLM EPA 600/R-93/116) •Asbestos Bulk Samples (TEM Semiquantitative) •Asbestos Air Samples (PCM NIOSH 7400) •Asbestos Air Samples (TEM NIOSH 7402) •150 - Mold Air Samples (Air Sample, Non-culturable) •Mold Bulk Samples (Total Fungal Structures) •VOC Scan Air Sample(EPA TO-17) •Lead paint chips (ICP or AA) •Formaldehyde Air Samples (NIOSH 2016) •Respirable silica (NIOSH 7500) •Welding Fume (OSHA 125G) f) Interested parties must complete the attached Business Declaration 5. Response Requirements All responses must be received via email at Jeffrey.Young@faa.gov no later than 3 PM (EST) on March 4, 2019.
