This is an amendment to a previously synopsized Request for Quotations (RFQ) for the National Park Service, Olympic National Park to remove, supply, and replace flooring throughout two office Suites: Cultural Resources and Animal House, located in the Headquarters Area, Olympic National Park, 600 East Park Avenue, Port Angeles, WA 98362. This Amendment no. 02 is necessary to extend the response date an provide three document revisions: Combined Synopsis Solicitation Full Text, Forms to Submit with Quote, and Statement of Work. Attachment 6, "Revised Combined Synopsis Solicitation Full Text," is changed as follows: Page 1 provides an updated list of solicitation attachments; Page 2 provides instructions to use the revised version of the Forms to Submit with Quotes which is now Attachment no. 7; Page 4 provides instructions for quoters to fill in provisions that have moved from Attachment 1 into Attachment 7; Page 11 provides an update to the instructions to quoters for what to include in quote. Quoters shall use Attachment 6, Revised Combined Synopsis Solicitation Full Text that replaces previous version that was Attachment 1. Attachment 7, "Revised Forms to Submit with Quote," provides a revised price schedule on Page 1 that changed the line items from being unit price per square foot to being lump sum price: one lump sum price for replacing non-carpet flooring with vinyl planks or similar as-quoted flooring in both the Animal House offices and Cultural Resources offices, and one lump sum price for replacing the carpeted areas with carpet tiles in both the Animal House offices and Cultural Resource offices. Page 2 provides revised fill-ins for quoters to include delivery time for vinyl plank flooring or delivery lead time for alternative as-quoted flooring, delivery lead time for carpet tile, and proposed project schedule start date. Pages 3 through 6 are the provisions where quoters will need to read through and fill in and submit with quote if applicable. Quoters shall use Attachment 7, Revised Forms to Submit with Quote that replaces previous version that was Attachment 3. Attachment 8, "Revised Statement of Work," provides additional text on Pages 3 and 4. Government is allowing quoters to submit quotes for non-carpeted flooring that in their professional experience may be better than installing vinyl plank flooring, so long as what is suggested is durable, waterproof, scratch resistant, thickness of 2.5 mm or greater. Government did market research to select vinyl plank flooring as the preference, however, quoter may have better alternative and if so, quoter will need to include product details, manufacturer information, warranty information, in quote. Refer to Attachment 6 for full details of evaluation factors that should be addressed in quote as well as following the instructions to quoters. Government not responsible to chase down any missing documentation that is not included in quote which could result in your quote not being further considered. Quoters shall use Attachment 8, Revised Statement of Work that replaces previous version that was Attachment 4. RESPONSE DATE IS EXTENDED by two business day, changed from "5 pm PDT August 19, 2022" to "5 pm PDT August 23, 2022." PAPER COPIES OF THE SOLICITATION WILL NOT BE ISSUED and telephone requests, email requests, or fax requests for the solicitation will not be accepted. New floor installation is anticipated to occur in September after the Labor Day holiday or October or November 2022, after the same office suites are newly painted under a separate contract. Sequence of work is painting first, then flooring. Coordination between the Contractor and NPS will be critical to time when work can be scheduled after painting. NPS will have removed all freestanding furniture from the offices prior to painting, which will stay out of the office areas where new flooring will be installed so contractor won't need to worry about scheduling around furniture being moved. There will be a pre-work meeting prior to starting any onsite work. The pre-work meeting will be scheduled within a few weeks from award. Onsite work can begin once the pre-work meeting has occurred, all submittals have been approved and products ordered and delivered, Contractor¿s liability insurance certificate is received, and the Contracting Officer has issued a Notice to Proceed letter. The Notice to Proceed letter will state the actual agreed upon period of performance start and end dates which will be a 30-day period. OTHER INFO. This project is for a service, not construction, being solicited and awarded as a total (100%) Small Business set-aside. The NAICS code for this solicitation is 238330 Flooring Contractors with an associated small business size standard of $16.5 million. The magnitude of for this project is less than $25,000.
