Amendment 0004 adds Site Pictures, Attachments 7, 8, 9, 10, and 11. Amendment 0003 answers Questions 10 and 11. Amendment 0002: dissolves small business set-aside restrictions; full and open without set-aside restrictions; sets new site visit/tour for 09/09/2019; and extends close/end/proposal due by date/time to 09/13/2019 1200 ET. Amendment 0001 adds: Attachment 4, Questions and Answers 01 through 09, and Attachment 5, Site Tour/Visit Vendor List. This Solicitation Notice is to announce the U.S. Department of the Interior (DOI), U.S. Geological Survey (USGS) Lansing Michigan, USA has a requirement for laboratory with hazardous materials (HAZMAT) cleanup services in accordance with the attached work statement, wage determinations, and HAZMAT price schedule. A copy of this solicitation may be accessed by clicking on the FedConnect Link or at www.fedconnect.net using Reference Number 140G0219R0036 and is being issued as a Request for Proposal (RFP). The policies contained in FAR Part 13, in conjunction with the policies and procedures for solicitation, evaluation and award prescribed in Part 15, as appropriate, shall be used for this acquisition. This opportunity is a 100% Small Business Set-aside; NAICS Code 562211 ENVIRONMENTAL SYSTEMS PROTECTION- ENVIRONMENTAL RE; small business size standard of $38.5M. A site tour has been scheduled for 08/15/2019 at 10:00 AM Lansing Michigan, USA Time. Firms interested in attending the tour shall email the Site Visit Coordinator, Julie Wolf, jawolf@usgs.gov and Cc the Contracting Officer, Don Downey, ddowney@usgs.gov by 1200 ET on the workday prior to the scheduled tour. The email shall include the Firm¿s Name and the name, phone number and email address of each tour attendee. Attendees shall be prepared to present a government-issued picture identification such as a state driver¿s license to access the facility and attend the tour. No additional, subsequent site visits will be arranged. All requirements for preparation, submission, and receipt of proposals contained in the solicitation shall be adhered to.
