Signed page 1 of the Standard Form 18 (attached), with your email and DUNS filled in. 2. Completed Section B, price grid (attached, begins on page 2) 3. Capability Statement, this is a statement that demonstrates that your firm can perform the work. This can be a written statement that says your firm is an existing BPA holder had has been awarded BPA calls that were successfully completed, provide your BPA number and a couple of BPA Call numbers. If you are a new firm that is interested in participating in this BPA action, provide a written statement that your firm can perform the work in the Statement of Work (attached, the 2 pages before the 55 pages of Wage Determinations at the end) and some proof that your firm can perform the work such as photos of the equipment owned by your firm that will be used in a BPA Call and if available, references from past customers where similar work was performed.
