THERE IS NO SOLICITATION AT THIS TIME. This request for capability information does not constitute a request for proposals. Submission of any information in response to this market survey is purely voluntary and will be used only for market research purposes to determine availability of sources, commerciality, and competitive strategy. The government assumes no financial responsibility for any costs associated with any response to this notice as incurred by prospective contractors. ****************************************************************************** Project Title: 140D0423Q0545 Surgical Cabinet Storage for Operating Rooms 23-1069-AER The anticipated Period of Performance or Delivery Date: 8/30/2023 The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 339113 ¿ Furniture, hospital, specialized (e.g., hospital beds, operating room furniture), manufacturing with a small business size standard of 800. Requirement Description: The vendor shall provide and deliver 6 Sets of cabinets for Operating Room suites at Northern Navajo Medical Center meeting the following specifications: Surgical Cabinet Storage for Operating Rooms Qty of 6 Cabinet Storage Sets- (1 Cabinet storage set for 6 different rooms) -Set of 3 cabinets that are all connected. Configuration of the cabinets shall be: 1 cabinet- two glass doors with 4 shelves, 36¿ wide. 1 cabinet- desk type with 4 drawers and overhead light 1 half glass door with 4 drawers -The set of 3 cabinets must fit within 8 Ft wide space and contain any connectors and side panels necessary for seamless professional look. The cabinet set has a Height maximum of 84¿. The cabinet set Depth requirement of 24¿. -Must be operating room grade materials consisting of Stainless Steel and glass. Required Delivery Date: 8/30/2023 Delivery, Installation and Assembly will be required. The government will evaluate market information to ascertain potential market capacity to provide equipment/supplies with those described in this notice. In accordance with the Buy Indian Act, 25 U.S.C. 47, the Indian Health Service shall give preference at all times, as far as practicable, to Indian economic enterprises. If your firm is capable of providing the supplies and/or services described in this notice, complete the attached representation form and submit it along with the rest of the requested documents identified in this notice. THE RESPONSES TO THIS SOURCES SOUGHT NOTICE/MARKET RESEARCH WILL BE USED TO DETERMINE THE ACQUISITION STRATEGY SUCH AS INDIAN-OWNED ENTERPRISE SET ASIDES, TOTAL SMALL BUSINESS SET ASIDE, ANY OTHER SOCIO-ECONOMIC SET ASIDE, OR UNRESTRICTED. NOTE: If this requirement is issued as a set-aside, FAR 52.219-14 Limitations on Subcontracting - Limitations on Subcontracting will apply; by submission of an offer and execution of a contract, the Contractor agrees that in performance of a contract it will not pay more than 50 percent of the amount paid by the Government for contract performance, excluding the cost of materials, to subcontractors that are not similarly situated entities. Any work that a similarly situated entity further subcontracts will count towards the prime contractor's 50 percent subcontract amount that cannot be exceeded. SUBMISSION INSTRUCTIONS: All interested sources must submit a capabilities package to the primary point of contact listed below, no later than (NLT) August 3, 2023 4:00 pm ED. AT A MINIMUM, THE FOLLOWING INFORMATION MUST BE SUBMITTED TO THE POC LISTED IN THIS NOTICE: 1. Confirm the Buy-Indian set-aside status you qualify for under following NAICS Code: 339113 ¿ Furniture, hospital, specialized (e.g., hospital beds, operating room furniture), manufacturing a. If the proposed NAICS is not the customary or applicable NAICS relative to the needs description, include your firm¿s proposed NAICS and rationale for the different NAICS. 2. Fill out the attached IHS Buy Indian Act Indian Economic Enterprise Representation Form. 3. If Non-Indian, indicate firm¿s size ¿ small or other-than-small; and other socio-economic program participation such as small-disadvantaged, 8(a)-certified, HUBZone, SDVOSB, and/or WOSB/EDWOSB 4. A positive statement of your intention to submit a quote to an upcoming solicitation as a prime contractor. 5. Provide firm¿s UEI Number. 6. Proof that your business is an authorized dealer of the make(s) and model(s) identified in this sources sought. 7. Information in sufficient detail regarding any previous experience on similar requirements. All of the above information must be submitted in sufficient detail for a decision to be made on availability of interested qualified Buy-Indian parties. If adequate interest is not received from Buy-Indian concerns, the solicitation may be issued as another type of set aside or unrestricted without further notice.
