SUBJECT: Solicitation Number 19AJ2021R0001 Life Insurance for Local Staff of the U.S. Embassy Baku, Azerbaijan Dear Prospective Offeror: The Embassy of the United States of America invites you to submit a proposal for customized Life Insurance for Local Staff of the U.S. Embassy Baku, Azerbaijan. The Embassy intends to conduct a pre-proposal conference, and all prospective offerors who have received a solicitation package are invited to attend. See Section L of the attached Request for Proposals (RFP). Please email your proposal only to BAKUBIDBOX@state.gov with a copy to BAKUBIDBOX@gmail.com on or before 18:00 (6:00PM) local (Baku) time March 15, 2021. No proposal will be accepted after this time. In order for a proposal to be considered, you must also complete and submit the following: 1. SF-33 (Blocks 15A, 16, 15B, 17, 18) 2. Section B, Pricing Schedule 3. Section B, Retention Amounts in B.3 and B.7 4. Section K, Representations and Certifications; 5. Additional information as required in Section L. 6. The Offeror shall submit a completed IRS Form W-14, found at www.irs.gov/w14, to ensure compliance with FAR 52.229-11. The Embassy of the United States in Baku will issue a solicitation for Life Insurance for Local Employed Staff in Baku, Azerbaijan. The estimate number of employees is 342. Anticipated performance is for one one-year base period and four one-year periods at the option of the Government. The Government plans to issue the solicitation in January, 2021 The solicitation will be issued electronically on beta.SAM.gov. No paper copies will be mailed. SAM registration is required pursuant to FAR provision 5.207, and prospective offerors are encouraged to register prior to the submittal of proposals.
