All vendors shall be sig... The Federal Bureau of Investigation (FBI) intends to procure commercial grade gym equipment for the office located in Detroit, Michigan. This RFQ falls under FAR Part 12 & 13. All vendors shall be signed up in SAM.GOV and have an active account to be considered for award. This is being competed as a Total Small Business Set-Aside, and only quotes from Small Businesses will be considered. Vendors shall quote all required products and attend the mandatory site visit to be considered for award. There will be a single award based upon Lowest Price Technically Acceptable. Delivery shall be performed within 60 days or less of order being placed. •Prices must be quoted for all 64 lines of equipment. •Inside delivery, unpacking, assembly and installation of products must be quoted. •Vendor must design a recommended layout for existing gym equipment and new equipment being quoted. •Site visit on Wednesday September 4th at 10am is mandatory. Any vendors that quote this requirement without having attended the mandatory site visit, will not be considered for award. o No later than 10am on September 2nd, please send the following information to Mary Lou Ludwig at mlludwig@fbi.gov. Provide her with the following information: •Name of the Company •Full name of all individuals attending the site visit. •Social Security numbers for all persons attending the site visit. •Places of birth for all persons attending the site visit. •Date of Birth for all persons attending the site visit. All vendors shall review, complete, and submit the Self Certifying Document (attachment number 1) and submit with their quote to be considered for award. If a vendor does not submit ALL of the required documents they will be considered nonresponsive to the RFQ.
