The Federal Bureau of Prisons (BOP), Federal Correctional Institution, Phoenix, Arizona intends to make an award of an indefinite delivery/requirements type contract with firm fixed prices to a responsible entity for the provision of Specialty Medical Services (Mammography & Ultrasound). All duties shall be performed in accordance with standards and methods generally accepted within the industry and in compliance with all Federal and State policies and regulations. The mobile mammography to include ultrasound will provide these services for the inmates incarcerated at the Federal Prison Camp (FPC), located in Phoenix, Arizona. The base period of performance shall begin on the effective date of award through a 12 month period, with the government's unilateral right to exercise up to four, 12 month option periods. The contractor shall perform an estimated four (4) sessions per year with a session consisting of a minimum twenty five (25) inmates and a maximum of thirty-five (35) inmates for 2-D Mammography and Ultrasound (as needed). The number of inmates examined per session is subject to vary for each visit depending upon the need of FPC Phoenix. The session(s) will be scheduled between the hours of 07:30 am to 3:30 pm, Monday through Friday excluding federal holidays. Contractor will provide all interpretations, diagnosis, and clinical recommendations and modalities in a report to the institution Health Services Department. The report shall be in typewritten or computerized format. Reports must be received within seven (7) days after performance of the aforementioned services.
