The Federal Bureau of Prisons - Field Acquisition Office in Grand Prairie, Texas, intends to make a single award for the provision of Optometry Services at the Federal Medical Center (FMC) located in Lexington, Kentucky. The contract type is an indefinite delivery/requirements contract with firm-fixed unit prices resulting from this solicitation. The contract period shall consist of a base period from effective Date of Award through 12 months from effective Date of Award. The estimated anticipated Date of Award is August 1, 2019. This requirement will also have 4 additional 12 months option years to be exercised at the unilateral discretion of the Government with a six month optional extension. All duties shall be performed in accordance and compliance with the Statement of Work. Contractor must be available to provide at least 16 and up to 40 sessions per week, depending on the needs of the institution. A session will equal one (1) hour. The Contractor is expected to an average of eight (8) sessions per day, 2 to 5 days per week. There will be an estimated 12-15 patients seen per eight hour clinic. Clinics will typically be scheduled between the hours of 7:00a.m - 3:30 p.m., with an option for an unpaid 30 minute lunch, Monday through Friday, excluding Federal Holidays. Total hours shall not exceed 40 hours per week. The scheduled days and hours will be mutually agreed upon by the Federal Government and the contractor depending on the needs of the institution. The North American Industry Classification System (NAICS) Code is 621320 (Size Standard: $7.5 million). This solicitation is being solicited as unrestricted. Faith-Based and Community Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. The solicitation is available on Monday, May 6, 2019 and will be distributed solely through the General Services Administration Federal Business Opportunities website at www.fedbizopps.gov .
