This notice does not constitute a commitment by the Government. The information gathered from market research will be vital in the formation and set aside status of the solicitation. The Bureau of Prisons has a future requirement for the provision of Optometry Services for the Federal Medical Center located in Lexington, Kentucky. All duties shall be performed in accordance with standards and methods generally accepted within the industry and in compliance with all Federal and State policies and regulations. The contractor will provide Optometry Services in accordance with the Statement of Work. The Government contemplates awarding of an indefinite-delivery, requirements-type contracts with firm-fixed unit prices resulting from the future solicitation. The contract period shall consist of a 12 month base period with four 12 month option periods for renewal and an optional extension period not to exceed 6 months. The Government anticipates requiring one 2000 optometry sessions per year. A session is defined as one hour. A Government solicitation document to provide these services is being developed, and a contract may or may not result. Market research is being conducted to determine interest and capability of potential sources for this requirement. DRAFT - Statement of Work Subject The Federal Bureau of Prisons (BOP), Field Acquisition Office, Grand Prairie, Texas, intends to award a single award, indefinite delivery, requirements type contract for the provision of an Optometrist(s) for the inmate population incarcerated at the Federal Medical Center (FMC) located in Lexington, Kentucky. Services provided under this contract will be on-site consultation and examination by an Optometrist. FMC Lexington is a Federal Bureau of Prisons facility that provides a medical mission for federal inmates. In meeting the medical needs of a growing inmate population, FMC Lexington provides essential medical care which includes Optometrist evaluation. Place of Performance The Optometrist(s) will provide onsite consultation and examination visits to FMC Lexington located at 3301 Leestown Rd., Lexington, Kentucky, 40511. Performance The services, being non-personal and professional in nature, will be provided in the method as considered appropriate in the local community and in line with Joint Commission standards. The performance of these services will also be verified by the designated contract monitor. Schedule of Performance Contractor must be available to provide at least 16 and up to 40 sessions per week, depending on the needs of the institution. A session will equal one (1) hour. The facility is expected to require an average of eight (8) sessions per day, 2 to 5 days per week. There will be an estimated 12-15 patients seen per eight hour clinic. Clinics will typically be scheduled between the hours of 7:00a.m - 3:30 p.m., with an option for an unpaid 30 minute lunch, Monday through Friday, excluding Federal Holidays. Total hours shall not exceed 40 hours per week. The scheduled days and hours will be mutually agreed upon by the Federal Government and the contractor depending on the needs of the institution. Due to the nature of the correctional setting, delays in performance of work may be expected. Such delays may include but are not limited to inclement weather, electrical or mechanical problems, or security matters. When contract employees report to the institution but are unable to perform services due to such delays, the contractor will be paid for one (1) session. Normally, any remaining sessions scheduled that day will be suspended and the contractor will be free to leave and the contractor will not be paid for remaining unperformed sessions. However, if the institution requests that the contractor remain at the institution for eventual performance, the contractor shall be paid for such sessions. In the event the contract employee will be absent for more than one day, the contractor will contact the institution and if necessary be responsible for providing replacement coverage for services. The replacement Optometrist shall meet all requirements as set forth in this Statement of Work. If the contractor fails to provide coverage, the Government may seek contractual remedy. Description of Duties The contractor shall provide to male and female inmates, on a non-discriminatory basis, routine refractions, eye examinations, fundoscopic exams, tonometry examinations, diagnosis, treatment, and prescribe appropriate medications allowed by law. He/she will provide and complete onsite all clinical documentation on all inmates examined as well as prescribe medication and prescribe and make adjustments on eye wear. If any eye pathologies are discovered, he/she will suggest appropriate referral services to the Primary Care Provider or Clinical Director. The contractor shall document all patient contact clinical notes appropriately and order prescription medication if necessary according to BOP policy and will use the Bureau Electronic Medical Record (BEMR) to do so. All clinical documentation is required to be completed in BEMR the day of examination. The contractor will be responsible for personal health care needs and cost associated with exposure, or possible exposure to any blood borne pathogen or other risk encountered during performance of this contract. The contract employee shall adhere to a standard of dress no less than business casual, avoiding khaki colored, fitted, low cut, revealing clothing. The contractor must also maintain malpractice liability insurance at all times and proof of this insurance must be kept on file with Health Services. The contractor shall agree to all regulations prescribed by the institution for safety, custody, and conduct of inmates. The contractor has sole responsibility and discretion for choosing and implementing the manner and method of performing tasks outlined in this Statement of Work as long as the requirements are met in accordance with the policies described. The services, although not directly supervised, shall be reviewed by the Contract Manager to ensure contract compliance. The Optometrist and contractor performance will be evaluated in accordance with FAR Part 42. Contact monitoring reports will be prepared and maintained in the contract file. In this evaluation, if the performance is deemed unacceptable, the Optometrist will no longer perform those duties and the contractor will be responsible for replacing the Optometrist. Qualifications/Credentialing The contractor shall provide an Optometrist candidate(s) which are a graduate of an accredited university, with a degree in Optometry, Board Certified with license to practice optometry in any state. The candidate must have a minimum twelve (12) months experience in Optometry. The contractor shall provide the name of the Optometrist candidate(s) that will be providing services for FMC Lexington and photocopies of documents evidencing their own and their candidate(s)' ability to meet qualifications to perform required services as stated in this SOW. Examples of evidence include a copy of license or permit, diplomas, internship/residency certifications, letters of recommendation, resumes, etc. Supplies Due to security reasons, all supplies and equipment that the contractor may need while at the institution, will be furnished by the institution. All records maintained by the contractor under this contract will remain the property of the institution. Inmate Management The contract employee shall not be responsible for the management of any inmates, other than to ensure inmates involved within the contract's scope of work abide by all rules to ensure their safety and well-being. The Optometrist shall complete Contractor Orientation prior to assuming institution involvement. Additionally, a "refresher" orientation must be completed annually. Reimbursement to the contract candidate for the time spent in the institution orientation or other specific security related training will be at the contract rate. The contract employee may, at any time, be removed from the institution at the discretion of the CEO. The contract employee is to constantly be aware of security and maintain a professional relationship with inmates at all times. Any violation of the institution rules and regulations are to be brought immediately to the attention of any Government employee employed at the institution. The Contract employee will report any infractions of rules and regulations by any inmate to the nearest employee, Health Services Administrator or the Contracting Officer's Representative (COR). Institution Security All contract personnel providing services within the confines of FMC Lexington shall have a complete background investigation conducted in accordance with BOP Program Statement 3000.03. The contractor/contract employees shall agree to all regulations prescribed by the institution for the safety, custody, and conduct of inmates. The contract employee will be required to operate a system of records as identified for medical treatment documentation, and will require adherence to the Privacy Act of 1974. The contract employee will be required to sign in and out of the institution for accountability purposes. A badge will be issued and worn at all times for proper identification. The contractor and all employees involved in the provision of services under this contract shall meet certain security requirements prior to the provision of services. The Health Services Administrator is responsible for conducting the appropriate vouchering and criminal background checks (National Crime Information Center [NCIC]). The contractor agrees to complete the necessary documents needed to complete the required security checks. An individual who does not pass the security clearance will be unable to perform services under the contract. If the resulting contract is to be awarded to an individual, all clearance procedures shall be applied prior to award. Individuals who do not pass the security clearance will not be awarded a contract. The final determination and completion of the security investigation procedures will be made at the sole discretion of the institution. Any individual deemed not suitable by the BOP requirements will not be granted access to perform services under the contract. This is a condition of the contract. The contractor must be in compliance with Department of Justice regulations at 8 CFR 274a regarding the employment of aliens. A copy of the directive is available upon the contractor's request. The Contractor, if an individual other than a corporation, upon contract award must complete several Human Resource Management forms. The forms are referenced as follows: (1) National Crime Information Center (NCIC) check; (2) Declaration of Federal Employment (OF-306); (3) FD-258 (fingerprint check); (4) Law Enforcement Agency checks; (5) Vouchering of Employers; (6) Employment Resume; (7) Completed Contractor Pre-employment Questionnaire; (8) Release of Information; (9) Questionnaire for Sensitive Positions (SF-85 for Low Risk, SF-85P for above low risk); (10) Credit Check, if applicable; (11) Urinalysis, if applicable. Additional Information This is a contractual agreement and not an appointment. All duties outlined in the Statement of Work shall be performed in accordance with the standards and methods generally accepted with the Contractor's field of expertise. Payment shall be based on the accomplishment of specific results, and the results to be obtained shall be entirely within the contractor's own unsupervised determination. In order to ensure accurate records of billing, the contractor must supply a monthly invoice/statement of service rendered. In order to ensure proper preparation of the monthly payment voucher, all hours worked must be documented by the contractor, showing the hours worked during each visit. Documentation will be provided on a "Log of Consultant's Visit". Payment terms are as specified in FAR 52.212-4(g), the due date for making an invoice payment by the designated payment office shall be the 30th day after the designated billing office has received a proper invoice, or the government accepts the services performed by the contractor. The contractor can however offer discount payment terms for prompt payments as specified in block 12 on the SF-1449 form of this solicitation. Half sessions will not be authorized for payment. Special Considerations The contractor shall immediately inform the COR in writing of any adverse action taken by any local, state, federal, or accrediting agency which affects the contractor's licenses or certifications. ***Please answer the following questions on a company letterhead** 1.
