Greetings Interested Vendors, The United States Coast Guard has a requirement to obtain behavioral modification services and support for its uniformed personnel. These services would support healthy weight management, healthy food choices, counseling, and other support for our uniformed personnel in accordance with the attached, draft, performance work statement (PWS). We are seeking commercial sources who have these capabilities to meet the full attached requirement. In addition to the attached PWS, the following information is requested from responding vendors: The NAICS is currently being determined among 541720, 812990 and 812191. 1) Does your product/services provide an APP for use on cell phone/smart devices? 2) The application/web sites/etc used to host and store USCG member data requires cybersecurity protection. The hosting environment must be Department of Defense (DOD) approved and certified to Impact Level 4, as identified in DOD Instruction 8510.01 (dated March 12, 2014 with incorporated change 3 dated December 29, 2020) and the DOD Cloud Computing Security Requirements Guide? FedRAMP High certification is also acceptable, but will require IT sponsorship to obtain DOD IL-4 certification and USCG Authority to Operate (ATO). Will your company meet these qualifications and/or be willing to engage in the ATO certification process? 3) Does your services/product currently have an ATO from DOD, or DHS, for use with sensitive but unclassified personally identifiable information (SPII) or DOD CUI (Critical Unclassified Information)? If so, can you provide a copy with your response? 4) Is your company currently registered in SAM.gov (www.SAM.gov) – required to do business with the federal government? If so, what is your DUNS Unique Entity ID # or your SAM Unique Entity ID #? 5) If not in SAM.gov, would you be willing to register to compete for a federal government award? 6) Have you previously performed work under federal government contracts? If so, please list the contract number, services/supply description, contract period and amount, and a current point of contact. 7) With your response, please include a capabilities statement tailored to the requirement. 8) What is your business size (according to the SBA and/or self-certification), to include any socio-economic category (8a, woman owned small business, etc?
