Description WALL-MOUNTED BLEEDING CONTROL STATIONS AND BLEEDING CONTROL TRAINING KITS This is a combined synopsis/solicitation for commercial item contract, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a separate written solicitation will not be issued. Solicitation number 70Z04221QQNF74600 applies, and is issued as a Request for Quotation – see attached for details. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-06 (JUL2021). This procurement is 100% set aside for small business. The North American Industry Classification System (NAICS) code is 423450 and the business size standard is 200 employees. U.S. Coast Guard intends to award a Firm Fixed Price Order. All responsible sources may submit a quotation, which if timely received, shall be considered by the Agency. Companies must have valid DUNS numbers and have an active entity registered with System for Award Management (SAM) @ www.sam.gov and shall provide the company Tax Information Number (TIN) with their offer. The solicitation document, incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2021-06. The following clauses and provisions apply to this acquisition: 52.212-1 Instructions to Offerors Commercial Items. The provision at 52.212-2 Evaluation Commercial Items; the evaluation criteria are as follows: (1) Price, (2) Estimated time required to complete work, (3) Past Performance. Evaluation factors (2) thru (3) combined are equal to (1). The award will be offered to the lowest-priced, technically acceptable (LPTA) offer made to the Government. In addition, all offerors must submit a completed form FAR clause 52.212-3, Offeror Representations and Certifications Commercial Items. This clause may be downloaded, free of charge, at http://www.acquisition.gov/far. Offerors who fail to submit a completed FAR 52.212-3 may be considered nonresponsive and will not be considered in the evaluation. All Offerors must be actively registered with the System for Award Management (SAM). Offerors who are not registered with SAM may be considered nonresponsive and will not be considered in the evaluation.
