U.S. Coast Guard (USCG) Office of Procurement and Contracting Formal Contracts Division (CG-9121) invites you to submit a proposal via the Acquisition of Commercial Items (Federal Acquisition Regulation [FAR] Part 12), to provide Multi-discipline Healthcare Services for USCG clinics as outlined in herein. This requirement is issued as a competitive 8(a) set-aside IAW FAR SUBPART 19.8. NAICS Code: 621111; Size standard: $12M The Solicitation no. 70Z023 22 R 11120001 is being issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-07. The North American Industry Classification System (NAICS) code that applies to this Request for Proposal (RFP) is 621111 (Office of Physicians (except Mental Health Specialists) and its size standard is $12M. DESCRIPTION: The USCG has a requirement for Multi-discipline Healthcare Services to be performed at various USCG clinics within the United States (CONUS), and outside the United States (OCONUS) and U.S. territories. The requirement is further described in the Performance Work Statement (PWS) Attachment 3. The various clinics are divided into three (3) regions. The USCG intends to award up to three (3), single award Indefinite Delivery Indefinite Quantity (IDIQ) regional contracts. It is anticipated that task orders will be awarded as Firm Fixed Priced orders. The contract minimum value for each IDIQ contract is $25,000.00. The contract maximum value for each IDIQ contract will be determined at time of award. The period of performance will be for 60 months ordering periods. The planned start date is 1 June 2023. The price proposal shall be submitted based on the CLIN structure in Attachment 1 - Pricing Spreadsheet.
