FAR 12.603(c)(2) Streamlined solicitation for commercial items. (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision for services necessary to perform maintenance services as defined in the Statement of Work (SOW). The Contractor shall be responsible for maintaining operational capability for all forklifts located at the Beeville, Texas FSA. This solicitation requires registration with the System for Award Management (SAM) in order to be considered for award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. Registration must be “ACTIVE” at the time of award. (ii) 70FBR620Q00000030 is the solicitation number and this solicitation is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-04 Dated 1/15/2020 see https://www.acquisition.gov/browse/index/far and https://www.acquisition.gov/hsar for clause language. (iv) This is a 100% Disaster Area Set-Aside for businesses residing and primarily doing business within the Presidentially Declared Disaster Area of DR-4332-TX, Hurricane Harvey in one of the Declared Counties listed within the solicitation attached. Further, the local disaster area set-aside must be within a 120-mile radius of the above address. (v) See the RFQ attachment for CLIN/pricing structure. (vi) Forklift Maintenance is required as stated in the specifications in the attached RFQ and the attached SOW. (vii) Date(s) are to be determined based off of date of award. The base period, option period 1, and option period 2 will be six (6) months each. *CLINs 0001, 0002, and 0003 (total combined average of 10 maintenance items price provided) will be added together for assessment of the lowest price. This will be repeated for each of the option periods, see the Price Schedule Table for completion. Ex. CLIN 0001 Hourly Rate Provided is $50.00, CLIN 0002 Hourly Rate Provided is $100.00, and CLIN 0003 Averaged Price of Maintenance Parts is $20.00, The total price evaluated in this example is $170.00 for the base period.
