FEMA is requesting a certified technician for a G25N Doosan forklift S/N FGA14-1330-06010 to keep the forklift in optimal working conditions providing maintenance and repairs. Maintenance services shall be performed as per attached Statement of Work. The Contractor shall provide a certified technician, equipment, tools, materials, supervision, testing gear, transport, and services necessary to achieve preventive maintenance on the forklift that will ensure the proper operation, either in continuous, intermittent, or standby status. The maintenance required will be on a quarterly basis. During each 6 months period of performance, two preventive maintenance services will be performed, one per quarter. The Contractor shall also provide emergency repair services as needed. When and if needed, funds would be added to this CLIN for services requested and rendered within estimated ceiling amount. Quote only estimated ceiling amount. The estimated ceiling required on CLINs 0002, 1002, 2002, and 3002 shall be based on price per labor hour ($____ x ___hrs = $______ 1 LO Total).
