This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation # - 47400PR250000003. This solicitation is issued as a Request for Quotation (RFQ). (iii) A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99. (iv) The North American Industry Classification System Code (NAICS) is 713940 and business size standard is $10 million. This is a total small business set aside. (v) Description of requirements for the items to be acquired- (i) Provide 180 gym/fitness memberships to include one dependent per membership to all vendor locations in Topeka and surrounding area. (ii) Provide list of gyms and their locations. Must have multiple locations in the Topeka area, i.e. More than one (1). (iii) Provide a list of pertinent amenities included with the gym membership. i.e. Fitness classes, mobile application, personal trainer, etc. (vi) Delivery Date – See period of performance.
