RFQ Due Date: 04/26/2024 Approximate Period of Performance (POP): 5-1-2024 to 7-01-2024 Description: Lead Inspection for US Coast Guard Housing Scope: Service is for Inspection taking samples and testing (no abatement) of lead in dust at the following address: U.S. Coast Guard Diamond Head Housing 3399 Diamond Head Road (Diamond Head Light House), Honolulu, Hawaii 96815 (1) Lead in dust sampling by, State of Hawaii Certified Lead-based Paint Risks Assessor(s) from the floor and window sill surfaces of each room of the following buildings: Main Quarters- estimated 13 rooms/lanai areas and 18 windowsills Main Quarters (enclosed patio/basement) - estimated 6 rooms/lanai areas and 1 windowsill Guest Quarters - estimated 5 rooms/lanai areas and 5 windowsills Quality control lead dust field blanks - estimated quantity 4 (2) Lead in dust analyses to be performed by AIHA accredited laboratory (1-week laboratory turnaround time). (3) Final survey report preparation and submittals of findings (no drawings).
