This is an advertisement for a Request for Quote (RFQ) to provide Podiatry services for the Wellpinit Service Unit (WESU) located in Wellpinit, WA. To help meet the critical needs of all Wellpinit Indian Health Center (aka David C. Wynecoop Memorial Clinic) patients, placing a high priority on diabetic and other podiatric needs, a contract will be issued to provide podiatry services, referrals to external providers is difficult. The WESU does not provide onsite housing and is 43 miles outside of Spokane, WA. RPMS is used within the facility, the “Period-of-Performance” is to be determined upon start date and will consist of Base plus four (4) Options Years. The vendor will need to submit a quote that covers estimated regular hour rate of 208 hours. Key Personnel consists of the Podiatrist. The Offeror shall submit resumes for the Key Personnel. All Key Personnel shall be a graduate of accredited Podiatry school, a minimum of three (3) years’ experience in the field of Podiatry, a current and unrestricted license in the State of Washington. All resumes must include the following information: name, years of experience with respective firm(s) and positions held – Active professional registration or relevant certifications; year first registered; if applicable – other experience and qualification relevant to the work required under this contract – list project in which the individual has worked to include the contract number, contracting agency, name of project(s), project location(s), start and end dates and project position if held different from the employment history. Evaluation of Factor for the Award will be made to the responsible firm whose quote is the most advantageous to the Government, qualification and State licensing. Other factors for this solicitation include key personnel and past performance for the quoter. Past performance information maybe be based on the Government’s knowledge of past and previous experience with the quoter or other basis.
