This is a Sources Sought Synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (Hubzone, 8(a), small, small disadvantage, veteran owned, women owned or service disabled veteran owned small business) relative to NAICS code 621512 (size standard $ 18 Million). Written responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this Sources Sought Synopsis, a solicitation announcement may be published in the GSA e-Buy or the Fedbizopps website. Responses to this Sources Sought Synopsis are not considered adequate responses to a future solicitation announcement. All interested offerors will have to respond to the solicitation announcement separately. The Indian Health Services, Phoenix Area Office of Acquisitions is seeking sources for potential contractor(s) to provide Diagnostic Imaging/Radiographer/X-Ray Technologist and Computerized Tomography (CT)/Diagnostic Radiologic Technologist or Ultrasound Technologist Services in Polacca, Arizona. Contractor shall identify their capability to perform this service and interest to provide qualified candidates per the Statement of Work (Attachment A). The intended contract period will be for a base year with the Government's option to extend up to four (4) years which is to estimated to begin 1 July 2021 . Response to this notice should include company name, address, point of contact, size of business pursuant to the following questions: (1) Is your business small (2) If small, does your firm qualify as a small, emerging business, or small disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act? (4) Is your firm a certified "Hubzone" firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned Small or veteran owned small business? (7) Is your firm able to provide qualified candidates willing to travel daily to and from Winslow, Arizona where lodging accommdations are? NOTE: Transient housing will be provided (at a daily rate) when available. If no transient housing, then the local hotel or housing in Winslow, Arizona. CURRENTLY and for the forecasted remainder of 2021 there is NO transient housing available. Responses to this notice shall be e-mailed to the attention of Ms. Michele Lodge, Department of Acquisitions, Phoenix Area Office, at Michele.Lodge@IHS.gov. Telephone responses will not be accepted. Responses must be received via e-mail no later than 3:00 p.m. PST. 24 February 2021. This notice is to assist IHS in determining sources only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. If an Offeror proposes to use any subcontractors, joint ventures, or teaming arrangements the Offeror shall include the percentage and type of work the subcontractor will accomplish to include their certifications. (Any subcontractors, joint ventures, or teaming partners shall have the required certifications as described in the sources sought notice) Important information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. No sub-contracting opportunity is anticipated.
