Each acquisition of supplies, services or construction that is subject to commercial items (FAR 12.209) or Simplified Acquisition Procedures (SAP) (FAR 13.106) must be set-aside exclusively for ISBEEs. Definition(s): INDIAN ECONOMIC ENTERPRISE (IEE) - Any business activity owned by one or more Indians, Federally Recognized Indian Tribes, or Alaska Native Corporations provided that: The combined Indian, Federally Recognized Indian Tribe or Alaska Native Corporation ownership of the enterprise constitutes not less than 51%; The Indian, Federal Recognized Indian Tribes, or Alaska Native Corporations must, together, receive at least 51% of the earnings from the contract; and The management and daily business operations must be controlled by one or more individuals who are Indians. DESCRIPTION OF REQUIREMENT The NAICS Code 621111 Offices of Physicians (except Mental Health Specialists) is applicable to this acquisition; the size standard is $14 million. The Contractor shall provide General surgery physician Services Crow Service Unit is comprised of 3 geographically distinct locations: Crow/Northern Cheyenne Hospital (CNCH) in Crow Agency, MT, Lodge Grass Clinic in Lodge Grass, MT and Pryor Clinic in Pryor, MT. General surgery services will be scheduled approximately 1-4 business days per month, depending on Service Unit needs and surgical program development, with the opportunity to expand up to 8 business days per month over the course of this contract. Hours of performance will be from 0730-1600, with a 30 minute unpaid lunch period. One Year performance with 4 Option Years.
