This is a combined synopsis/solicitation for Non-Personal Service Contracts for the Registered Nursing Emergency Department prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested. The solicitation number RFQ1464304A is issued as a request for quotation (RFQ). The Navajo Area Indian Health Service (IHS), Gallup Service Unit (GSU) intends to award a firm-fixed price Purchase Order. The Government reserves the right to not make an award, if necessary. Period of Performance: The Government will award a firm fixed priced contract from date of award through 12/31/2020. The Contractor shall provide an all-inclusive rate, and is responsible for all costs associated with providing said services, i.e. travel, per diem, lodging/housing, etc. The Contractor shall be responsible for housing respective providers as there are no government quarters available. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-01, effective October 01, 2020. The RFQ is issued with the NAICS code 561320 – Temporary Help Services, with a small business size standard of $30.0 dollars. Provide proposed rates in the Attachment No. 1. PROVIDE A RATE THAT WILL ALLOW YOU TO RECRUIT PROVIDERS THROUGHOUT THE CONTRACT PERIOD. The Government will consider competitive rates upon negotiation, and will obligate funding on a quarterly basis. 2. Provide complete Candidate Profile: Curriculum vitae, resumes, licensures, and all relative documents for proposed candidates. 3. Provide evidence of Provider's Experience of comparable scope and complexity in providing services within the past three (3) years. 4. Provide evidence of the Contract Company's Past Performance of comparable scope and complexity in providing services within the past three (3) years.
