This is a Firm Fixed-Price, Non-Personal Service acquisition for Medical Physicist Services for the IHS Pine Ridge Hospital located in Pine Ridge, South Dakota. Contractor is to perform according to the Statement of Work (SOW). The “unit price” is an all-inclusive cost. Period of Performance: Physical survey must be conducted during May of each year and reports are due within 30 days of completion. Assistance to create corrective action plan, review plan with program director, and re-evaluate to ensure compliance is acceptable may occur within the following 11 months. Total Period of performance is a 1-year contract.
