This is a 100% Total Small Business Set-Aside Solicitation Request for Quote (RFQ). The Phoenix Indian Medical Center (PIMC), located in Phoenix, Arizona, is seeking Small Business Vendors that can provide a quantity of three (3) IQ View License Software with support required to perform the software implementation as described in the attached Scope of Work (SOW). Offerors are to submit a Technical Proposal, a Price Quote to include base plus 4 option years to renew for support, and Past Performance in order to be considered. Offerors submission information can be found in Attachment B - Provision and Clauses. The Government intends to award one base year with 4 option years. A firm, fixed price award is anticipated. Award to be determined by meeting specifications and Lowest Price Technically Acceptable. Quotes are all inclusive. See attached documents for additional terms and conditions.
