The Billings Area – Crow Service Unit, Indian Health Service (IHS) is requesting quotes in response to RFQ-CSU-21-014-RDJ for Bi-Annual Preventive Maintenance (PM) services for the Crow Service Unit, Crow/Northern Cheyenne Hospital, Crow Agency, Montana, 59022. The CSU/IHS will be awarding a fixed price contract to the successful offeror for the services as provided in the scope of work. This procurement is being conducted under Simplified Acquisition procedures pursuant to the authority of FAR Subpart 13.2, Simplified Acquisition Procedures; FAR 37.101, Service contract. The solicitation is full and open competition. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-05. The associated North American Industry Classification System code is 811219 and the small business size standard is $22.0 million. Potential offerors may submit an offer by including a rate for the base year and each option year. PRICE SCHEDULE – BI-ANNUAL PREVENTIVE MAINTENANCE SERVICES, CROW SERVICE UNIT. Proposals must be inclusive of transportation, meals, insurance, per diem and lodging. BACKGROUND/PURPOSE: The Crow Service Unit (CSU) is requesting a base plus four option year service contract that includes all manufacturer recommended preventative maintenance checks, inspections, and calibrations, as well as required periodic maintenance parts and supplies such as filters and oxygen sensors, for the two (2) anesthesia units currently in service. Service is to be performed on site by a GE factory trained/certified technician with the anesthesia units remaining in the surgical suites; provide certification of training with submittal. Anesthesia PM services ensure the anesthesia machine is working properly for patient safety. This encompasses the monitors, modules, cables, digital vaporizers, as well as the actual anesthesia machine. This will require the servicing technician to don appropriate PPE attire for entry to the surgical suites, such as KLEENGUARD coveralls, disposable shoe covers and a head cover. PPE will be provided by the Crow Service Unit. Service reports including any and all maintenance data, calibration data and parts/supplies used will be provided to the CSU Anesthesia Supervisor and the Biomedical Equipment Support specialist within 5 business days of the service visit. POTENTIAL ADDITIONAL WORK ITEMS: This request is for Preventive Maintenance. However, repair problems may be discovered. Any repairs must be completed by a technician that is currently a GE certified technician for the anesthesia machines and monitors. They must be able to repair the machines, upload all new updates to the software, and replace faulty parts with OEM parts to maintain the warranty. Any repair work will be reviewed and approved by the Project Manager. Any changes to the contract will be issued by the Contracting Officer in the form of a modification to the contract. The Contracting Officer is the only person who can make any change to the contract. STATEMENT OF WORK Statement of Work (SOW) for the manufacturer recommended bi-annual preventative maintenance (PM) required for two (2) GE Aisys CS-2 surgical anesthesia units located at the Crow / Northern Cheyenne Indian Health Service Hospital. The GE anesthesia machines consist of the following components: GE Carescape Patient Data Module (PDM), GE Carescape B850 patient monitor, GE Aisys anesthesia machine, and E-Scaio-00 Respiration module. Anesthesia unit #1 serial number is APWX01402. Anesthesia unit # 2 serial number is APWW01645.
