The Billings Area – Crow Service Unit, Indian Health Service (IHS) is requesting quotes in response to RFQ-CSU-21-013-RDJ for a Site Survey and Network Assessment for the Crow Service Unit, Crow Agency, Montana, 59022. The CSU/IHS will be awarding a fixed price contract to the successful offeror for the services as provided in the scope of work. This procurement is being conducted under Simplified Acquisition procedures pursuant to the authority of FAR Subpart 13, Simplified Acquisition Procedures; FAR 37.101, Service contract. The solicitation is set aside 100% for small business concerns. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-05. The associated North American Industry Classification System code is 541512 and the small business size standard is $30.0 million. This contract includes the Service contract Act, Wage determination No. 2015-5397, Revision No: 12, Dated: 12/21/2020. Wage rates are included. STATEMENT OF WORK BACKGROUND/PURPOSE: A Survey and Network Assessment is needed to determine the next steps required to implement future upgrades that may include but are not limited to: Removing outdated network cables, new network cable drops, hardware upgrades, wire management, network efficiency, and other recommended improvements. SCOPE: Perform site survey for Crow/Northern Cheyenne Hospital and up to quantity two (2) satellite clinics. SEE ATTACHED FULL RFQ.
